| 目的 探讨恶性黑色素瘤的全身18F-FDG PET/CT影像表现特点,评价18F-FDG PET/CT对恶性黑色素瘤分期的价值。方法 对28例病理证实的恶性黑色素瘤患者的18F-FDG PET/CT全身显像资料及临床资料进行回顾性分析。结果 眼球内黑色素瘤表现为眼球内高密度结节,>1.0 cm病灶FDG代谢增高,PET/CT较单独PET或单独CT发现更多转移灶,但对于原发病灶的显示较MRI差。颅内恶性黑色素瘤PET/CT表现有一定特征,但对病灶的检出及鉴别诊断较MRI差。四肢恶性黑色素瘤原发灶显示为局部软组织肿胀,FDG摄取未见明显增高;转移灶显示为局域淋巴结转移及远处转移,单独CT对所有检出病灶诊断的阳性率为87.20％,单独PET对所有检查病灶诊断的阳性率为72.5％,PET-CT对所有检查病灶诊断的阳性率为99.1％。结论 18F-FDG PET/CT对于恶性黑色素瘤的定性诊断有一定价值,对判断临床分期具有重要价值。PET-CT检查较单独CT或单独PET检查能够检出更多病灶,提高诊断阳性率。 |

| Objective To discuss the 18F-FDG PET/CT features of melanoma, and to evaluate its value in staging of melanoma. Methods 18F-FDG PET/CT images and clinical data of 28 patients with melanoma proved pathologically were retrospectively analyzed. Results Lesions of choroidal melanoma in the eyes presented with hyperdensity, and metabolism increased in >1.0 cm lesions. PET/CT found more lesions than PET or CT, but inferior to MRI in displaying primary lesions. Intracranial melanoma had some characteristics on PET/CT, but MRI was better than PET/CT in detecting and differentiating these lesions. Local edema in soft tissues without uptake of 18F-FDG appeared in primary melanoma in the extremities. Local and remote metastasis could be found in whole body using 18F-FDG PET/CT. The frequency of privately CT and privately PET were 87.20％ and 72.50％. Conclusion 18F-FDG PET/CT has some value in diagnosis and staging of melanoma. |
