      目的 采用多排螺旋CT同层动态对比增强技术研究对比剂在胸部血管中的药代动力学过程。 方法 将62例患者按对比剂注射速率不同随机分为3组:3.0 ml/s、4.0 ml/s、5.0 ml/s,对比剂总量均为40 ml,测量升主动脉、肺动脉干强化峰值、峰值时间以及各自达到200 HU的时间(Tb200),绘制升主动脉、肺动脉的时间-密度曲线(TDC)并计算主-肺动脉峰值时间差,统计分析三组间数据。第4组12例患者对比剂注射速率为3.0 ml/s,对比剂总量为90 ml,绘制肺动脉及主动脉的时间-密度曲线。 结果 ①第1~3组肺动脉干、主动脉的强化峰值、峰值时间和肺动脉Tb200差异均有统计学意义(P<0.05);第1~3组内肺动脉强化峰值均高于主动脉强化峰值(P<0.05);第1~3组间主-肺动脉峰值时间差异无统计学意义(P>0.05);第3组肺动脉可见较明显的对比剂再循环。②第4组肺动脉和主动脉TDC曲线可见"三个期"和两个"拐点"。 结论 ①随着注射速率的提高,对比剂在血管内的峰值提高,峰值时间提前,但对比剂通过肺的平均时间不变;②胸部血管内对比剂峰值及峰值时间在常规剂量条件下由注射时间和对比剂流动时间来决定。

      Objective To investigate the process of pharmacokinetics of contrast media in thoracic angiography with dynamic MDCT scan. Methods Sixty-two patients were randomly assigned to simple bolus injection protocols, i.e. 40 ml of a nonionic contrast agent (300 mgI/ml) injected at flow rate of 3.0 ml/s, 4.0 ml/s and 5.0 ml/s, respectively. The peak enhancement values, the peak enhancement time and the time up to 200 HU (Tb200) of pulmonary trunk and ascending aorta were measured. TDC of pulmonary trunk and ascending aorta were drawn, and the time between peak pulmonary trunk enhancement and peak ascending aorta enhancement were calculated. All data among three groups were statistically analyzed. There were 12 patients in the fourth group with 90 ml bolus injection of contrast media at 3.0 ml/s. TDC of pulmonary trunk and ascending aorta were drawn and analyzed. Results ①The peak enhancement values, the peak enhancement time of ascending aorta and pulmonary trunk and Tb200 of pulmonary trunk among three groups were significant different (P<0.05). The peak enhancement values of pulmonary trunk were higher than those of ascending aorta (P<0.05) in three groups, and no significant difference of the time between peak pulmonary trunk enhancement and peak ascending aorta enhancement was found among three groups (P>0.05). The recirculation of contrast media occurred in the group of 5.0 ml/s flow rate was more significant than that of two others; ②There were "three stages" and "two bumps" on TDC of pulmonary trunk and ascending aorta in the fourth group. Conclusion ①With increasing injection rate, the peak enhancement values of pulmonary trunk and ascending aorta increase. Fast injection rate with the same total dose brings higher peak enhancement at earlier times, but the mean transit time of contrast media through lung is constant; ②The peak enhancement values and the peak enhancement time of ascending aorta and pulmonary trunk are determined by the relative contribution of injection duration and contrast media traveling time.
