| 目的 评价B-flow技术对不同糖耐量水平者颈动脉粥样硬化的诊断价值。 方法 将87例受试者根据糖耐量不同分为3组:正常糖耐量(NGT)组28例,糖耐量减退(IGT)组33例,糖尿病(DM)组26例。运用B-flow技术对不同糖耐量水平者的颈动脉内中膜厚度(IMT)及粥样硬化斑块形成情况进行检测,并比较B-flow与传统二维显像及CDFI在斑块检出率上的差异。 结果 DM组的IMT测值、斑块发生率及斑块积分明显高于其他两组(P<0.05);IGT组的IMT测值、斑块发生率及斑块积分明显高于NGT组(P<0.05)。各组检出斑块中软斑所占比例高于混合斑及硬斑,差异有统计学意义(P<0.05);各组间斑块类型所占比例差异无统计学意义(P>0.05)。B-flow对软斑块的检出率优于二维超声及CDFI。 结论 IMT及颈动脉粥样硬化情况与不同糖耐量密切相关;运用B-flow技术检测不同糖耐量者的颈动脉粥样硬化情况较传统方法方便、可靠,具有重要的诊断价值。 |

| Objective To assess the diagnostic value of B-flow technique in detecting carotid atherosclerosis with varying degrees of glucose tolerance. Methods A total of 87 subjects were divided into three groups according to OGTT, i.e. 28 subjects with normal glucose tolerance (NGT), 33 subjects with impaired glucose tolerance (IGT), and 26 patients with type 2 diabetes mellitus (DM). The intima-media thickness (IMT) and the atherosclerotic plaques of carotid artery were observed. The detection rate of soft plaque was compared among B-flow, two-dimension and CDFI. Results The IMT of carotid arteries, the plaque score and the detection rate of plaques in DM group were higher than those in IGT group and NGT group (P<0.05), while the above indices in IGT group were higher than those in NGT group (P<0.05). The detection rate of soft plaque was higher than hard plaque and mixed plaques in all groups (P<0.05). There was no difference in the detection rate of different plaques among three groups (P>0.05). Compared with two-dimension and CDFI, B-flow has an advantage in the detection rate of soft plaque. Conclusion IMT is significantly correlated with glucose tolerance and atherosclerotic plaques. Compared with traditional methods, B-flow technique is a convenient, efficient and valuable method to detect carotid atherosclerosis in subjects with varying degrees of glucose tolerance. |
