       目的 应用64排螺旋CT自动管电流调节技术对重症肺部感染的患儿进行胸部扫描,探寻儿童胸部CT低剂量扫描的临床可行性。方法 选取26例临床治疗前后需进行胸部CT检查对比的重症肺部感染的患儿(中位年龄3.2岁),治疗前(对照组)应用常规固定管电流技术:120~200 mA,治疗后(试验组)应用Z轴自动管电流调节技术:预设噪声指数根据年龄段设置范围为8~12 HU。分别记录CT剂量指数(CTDIvol)。对图像质量采用5分制进行评估,并用Kappa检验评价观察者间的一致性。结果 对照组和试验组的CTDIvol分别为(7.13±1.55)mGy和(2.59±1.06)mGy,试验组CTDIvol比对照组降低了63.60%。试验组和对照组平均图像诊断接受率评分分别为3.18和4.60,所有图像均符合临床诊断要求,观察者间的评估具有一致性。结论 在获得稳定图像质量的前提下,儿童胸部64排螺旋CT自动管电流调节技术扫描可有效减低射线剂量。
英文摘要:
      Objective To evaluate the dose reduction potentials for children with severe pulmonary infection with automatic tube current (mA) modulation on 64-slice MDCT. Methods Twenty-six children (mean age 3.2 years) underwent chest scans with GE 64-slice VCT with or without automatic tube current (mA) modulation technique before and after treatment, then image quality and radiation dose for these two groups were compared. The noise index (NI) for the study group (after treatment, scanned with automatic tube current modulation technique) was set to 8-12 HU (age dependent), while NI for the control group (before treatment) was set fixed-mAs (range from 120 to 200 mA). CT dose index of volume (CTDIvol) were recorded for both groups. Image quality was assessed by two pediatric radiologists on a 5-point scale, scores greater than or equal to 3 which were considered clinically acceptable. The degree of interobserver concordance was evaluated with calculation of Kappa statistics. Results The average CTDIvol for study group and control group was (2.59±1.06)mGy and (7.13±1.55)mGy, respectively. The average CTDIvol with the use of NI was about 63.60% lower than that with the fixed mAs setting. The mean image quality score for the study group and control group was 3.18 and 4.60, respectively, indicating all studies had acceptable image quality, and there was inter-observer agreement in diagnostic acceptability. Conclusion The automatic tube current modulation method may be used to obtain consistent image quality which can reduce radiation dose for children undergoing 64-slice MDCT chest scans.
