      目的 分析隆乳材料及隆乳术后并发症的MR及全数字化乳腺钼靶摄影(FFDM)表现,观察MR及FFDM在乳腺假体植入术后评价中的价值。方法 收集43例经MR和(或)FFDM检查的隆乳术后患者,分析不同类型假体的MR及FFDM表现。结果 ①水凝胶假体33例,MR明确显示66只乳房内液态假体的信号、形态、位置、假包膜及向周围组织渗漏情况;FFDM检查显示液态假体受腺体类型、注射位置、注射量及渗漏情况影响较大,部分病例显示不清。②自体脂肪3例:MR显示其中3只乳内见脂肪信号,1只乳感染并脓肿形成。FFDM显示6只乳房内见不同程度和形态的钙化;③硅胶假体3例:MRI可清楚显示其包膜及内部结构;④硅胶/水凝胶双假体4例:MR显示其中7只假乳包膜挛缩,周围散在液态假体信号。FFDM对两种假体内部及边缘情况显示欠佳。结论 与FFDM比较,MR是评价乳腺假体植入术后情况的优选方法。

      Objective To observe MRI and FFDM appearances of breast implants and postoperative complications, and to assess the clinical value of MRI and FFDM. Methods Forty-three patients after augmentation mammoplasty underwent FFDM and MRI, the imaging manifestations of different replacements were analyzed respectively. Results ①In 33 patients after polyacrylamide hydrogel injection, MRI definitively showed the signal, shape, location, pseudo-capsule and peripheral leakage of liquid implants in all 66 breasts, while FFDM manifestations were influenced by the types of mammary glands, injected location, dose and leakage; ②In 3 patients after auto-fat injection, fat signal was seen in 3 breasts and abscess was revealed in 1 breast on MRI, and FFDM showed different calcifications in 6 breasts; ③In 3 patients after silicone implants, MRI showed the margin and capsule contracture of implants clearly; ④In 4 patients after both silicone and polyacrylamide hydrogel replacements, MRI revealed that the capsule contracture shrunk in 7 breasts, surrounded by split polyacrylamide hydrogel, whereas the inner and bordering conditions remained unclear on FFDM. Conclusion Compared with FFDM, MRI is a superior method in accessing patients with augmentation mammoplasty.
