      目的 探讨CT和MR在诊断外耳道腺样囊性癌中的应用价值。方法 7例经组织病理学证实的外耳道腺样囊性癌患者均接受CT检查,4例接受MR检查,分析CT、MR表现并与病理结果对照。结果 7例均为单耳发病,左耳3例,右耳4例;表现为外耳道局部隆起1例、肿块6例;形态规则1例、不规则6例;累及软骨部3例,软骨部与骨部均受累3例,颞骨及耳周广泛受累1例;侵犯中耳2例、腮腺2例、颞颌关节1例;骨质破坏5例,骨质受压1例,无骨质改变1例;T1WI呈低信号3例,等信号1例,T2WI均为高信号,增强扫描呈不均匀强化3例,均匀强化1例。结论 外耳道腺样囊性癌患者术前行CT和MR对该肿瘤定位、定量诊断有重要价值。
英文摘要:
      Objective To investigate the clinical applications of CT and MRI in diagnosing adenoid cystic carcinoma (ACC) of the external auditory canal (EAC). Methods CT scanning was performed in all 7 patients with ACC of the EAC confirmed by histopathology, and 4 patients received MR scanning. All imaging data of CT and MR were compared with histopathologic findings retrospectively. Results ACC were found in the left EAC in 3 and in the right EAC in 4 patients. Masses of the EAC were displayed in 6 patients, and swelling of local soft tissue was detected in only 1 patient. ACC of the EAC was regularly shaped in 1 patient, but irregularly shaped in 6 patients. ACC located in the chondral part of EAC in 3 patients, in both the osteal and chondral part of EAC in other 3, and extended almost the whole temporal bone and spread around the ear in the rest one. The lesions extended into the middle ear in 2 patients, the parotid gland in 2, and the temporomandibular joint in 1 patient. Bone destruction was found with CT in 5 patients, bone compression in 1, and no bone involvement in 1 patient. MR T1WI demonstrated hypointense signal in 3 patients and isointense signal in 1, MR T2WI showed hyperintense signal in all 4 patients. Postcontrast MR imaging displayed heterogeneous contrast enhancement in 3 patients and isogeneous enhancement in the rest one. Conclusion CT and MRI can quantitatively localize and diagnose ACC of EAC before surgery.
