| 目的 探讨成人甲状舌管囊肿(TDC)的超声图像特征及术前超声诊断TDC的价值。 方法 超声检查100例TDC,共101个病灶,通过以下方面评价肿物特征:病灶位置、大小,囊壁、边界、形状、囊内回声、后方回声增强、囊内分隔、瘘管以及是否存在甲状腺,并将超声表现与术后病理结果进行对照。 结果 101个病灶TDC,最大径线12~50 mm。60个TDC (59.41%)位于颈部中线处,41个(40.59%)偏于一侧;舌骨上方6个(5.94%),舌骨表面17个(16.83%),舌骨下方73个(72.28%),甲状腺峡部5个(4.95%)。47个(46.53%)TDC表现为典型的薄壁囊肿,45个(44.55%)壁厚,9个病灶囊壁不明显。所有病灶均边界清晰,17个(16.83%)可见分叶。囊肿内部可呈无回声(28/101,27.72%)、均匀低回声(34/101,33.66%)、类实性回声(22/101,21.78%)及不均匀混合回声(17/101,16.83%)。87个TDC (86.14％)可见后方回声增强。 结论 成人TDC有多种超声表现,其超声特征与急性感染、慢性炎症及出血之间无明确联系。 |

| Objective To observe the ultrasonic image characteristics of thyroglossal duct cysts (TDC) in adults, and the diagnostic value of ultrasound before operation. Methods Sonograms of 100 patients (101 lesions) with TDC were reviewed. The lesions were observed on location, size, wall thickness, margins, loculation, internal echogenicity, posterior enhancement, presence of septa, solid component, fistulous tract, and presence or absence of the thyroid. The ultrasonic manifestations then were compared with the pathologic findings. Results Among 101 TDC, 60 (59.41%) located in midline, 73 (72.28%) infrahyoid, and 87 (86.14％)with posterior echo enhancement. Typical thin wall was found in 47 (46.53%) lesions. Anechoic internal echogenicity (28/101, 27.72%), homogeneously hypoechoic with internal debris (34/101, 33.66%), pseudosolid (22/101, 21.78%) and heterogeneous (17/101, 16.83%) were detected with ultrasound. Conclusion TDC in adults has various ultrasonic manifestations. No certain correlation exists between ultrasonic manifestations and to pathologic findings including acute infection, chronic inflammation and hemorrhage. |
