| 目的 应用实时三维超声心动图(RT-3DE)技术评价继发孔型房间隔缺损(S-ASD)患者的心房容积和功能,以及外科修补术对心房容积和功能的影响。 方法 应用RT-3DE技术测量30例S-ASD患者、14例S-ASD外科手术患者和20名正常人的左、右心房的最小容积、最大容积及收缩前容积,并计算左、右心房传导功能和泵血功能。分别比较S-ASD组与正常组,S-ASD组术前与术后结果。 结果 S-ASD组左、右心房的最大、最小及收缩前容积较正常组增大;左、右心房的主动排空容积较正常组增大,并与分流量(Qp/Qs)呈显著相关(P＜0.05)。但其主动排空容积分数与正常组差异无统计学意义,而右心房的被动排空容积分数较正常组减低。外科修补术后,右心房的最大、最小及收缩前容积较术前减低,而左心房最大、最小及收缩前容积较术前增加。左、右心房主动排空容积分数和被动排空容积分数均较术前减低。 结论 RT-3DE能测量并评价S-ASD患者的心房容积和功能。S-ASD患者心房容积增大,右心房传导功能减低,泵血功能未受影响;S-ASD患者外科修补术后右心房容积变小,左心房容积变大,双房的传导功能和泵血功能均减低。 |

| Objective To quantify the volume and function of atrium in the patients with secundum atrial septal defect (S-ASD) with real-time three-dimensional echocardiography (RT-3DE), and to evaluate the influence of surgical closure on the volume and function of atrium. Methods RT-3DE data of 30 patients with S-ASD and 20 normal controls were acquired and analyzed. Minimum volume (Vmin), maximum volume (Vmax) and pre-contraction volume (Vpre) of atrium were measured and the conduit function and pump function were calculated. All the results were compared respectively. Results Vmin, Vmax and Vpre of left and right atrium in the patients with S-ASD were larger than those in the controls. The active emptying volume (Va) of left and right atrium were larger, and correlated with Qp/Qs significantly, but the active emptying fraction (Fa) were significantly different between the two groups. The passive emptying fraction (Fp) of right atrium was smaller than that in controls. After surgical S-ASD closure, the Vmin, Vmax, Vpre of right atrium became smaller, while those of left atrium became larger. The Fa and Fp of left and right atrium both became smaller. Conclusion The atrial volume and function in the patients with S-ASD can be evaluated with RT-3DE. The atrial volume enlarges in the patients with S-ASD, and the conduit function decreases, but the pump function is not affected. After surgical S-ASD closure, the volume of right atrium diminishes, volume of left atrium enlarges, while the conduit function and pump function of both atria reduce. |
