目的 探讨急性缺血性脑梗死患者发病24 h内弥散各向异性变化的规律及其临床应用价值。方法 回顾性分析33例次发病24 h内脑梗死病灶的弥散张量成像(DTI)资料,计算病灶和对侧相应部位的平均各向异性分数(FA)和平均弥散系数(ADC)。采用配对t检验的方法分别比较不同发病时间段的梗死病灶与对侧相应部位的FA和ADC值。结果 发病24 h内梗死病灶的ADC值显著低于对侧(P<0.01);而病灶平均FA值与对侧比较差异无统计学意义(P=0.74),6 h内、6~12 h以及12~24 h内病灶与对侧比较差异也无统计学意义(P>0.05)。急性期内病灶FA值变化存在显著个体差异,FA升高和保持不变者在三个时间组内发生率差异无统计学意义(P=0.73)。结论 发病24 h内脑梗死病灶弥散各向异性与对侧比较差异无统计学意义,但个体间存在明显差异,这种个体差异有望为脑卒中患者的个体化治疗方案的选择提供依据。 |

Objective To evaluate the characteristics of diffusion anisotropy of acute stroke within 24 h from the initial stroke onset. Methods Data of 33 diffusion tensor imaging of 27 patients within 24 h after onset of ischemic were retrospectively analyzed. Fractional anisotropy (FA) and average diffusion coefficient (ADC) were calculated using an off-line computer, while values of FA and ADC were measured in the lesions and contralateral regions. The FA and the ADC values of the lesions were compared with those of the contralateral regions. Results ADC values of the ischemic lesions were significantly lower than those of the contralateral regions (P<0.01). There was no significant difference of mean FA values between lesions and the contralateral regions (P=0.74), nor among patients within 6 h, 6-12 h and 12-24 h (P>0.05). Significant individual difference was found in changes of mean FA value during acute stage, but not in the occurrence of elevated or unchanged FA among patients within 6 h, 6-12 h and 12-24 h (P=0.73). Conclusion There is no significant difference in diffusion anisotropy between lesions and the contralateral regions within 24 h, whereas individual difference is significant. Individual difference of diffusion anisotropy of acute stroke within 24 h may be helpful to the choice of individual therapeutic schemes of stroke. |
