       目的 探讨扩散张量成像(DTI)对诊断缺氧缺血性脑损伤早期脑水肿的应用价值及其与AQP-4的相关性。方法 制备新生猪缺氧缺血性脑损伤模型30头,按缺氧缺血后时间2,6,12,24,48,72 h平均分为六组;假手术组5头。观察两组各时间点的DTI及AQP-4表达的时程变化。结果 在新纹状体区域,细胞水肿持续至72 h,ADC值减低,在24 h达最低值,与对照组相比差异有统计学意义(F=27.42,P<0.05);ADC值与AQP-4免疫组化OD值变化呈线性正相关(r=0.875,P<0.05)。在周围脑实质区域,缺氧缺血后6 h出现细胞毒性水肿,而后至72 h血管源性水肿占主导,ADC值短暂下降后升高,48 h达到最高值并与对照组相比差异有统计学意义(F=50.25,P<0.05);ADC值与OD值变化趋势一致,但差异无统计学意义(r=0.505,P>0.05)。FA值逐渐减低。结论 DTI能够监测缺氧缺血性脑损伤早期脑水肿的变化过程;ADC值变化与AQP-4表达变化相关。

      Objective To assess the value of DTI in the early stage of brain with hypoxic-ischemic (HI) injury and the correlation between DTI and AQP-4. Methods Thirty hypoxic-ischemic injury models in newborn piglets were performed and evenly divided into six groups by time points (2, 6, 12, 24, 48, 72 h) after HI injury, while 5 newborn piglets underwent sham operation as control group. At each time point, DTI and AQP-4 expression were observed and compared. Results In neostriatum, cytotoxic edema continued for 72 h and reached minimum at 24 h with decrease of ADC values at different time points (F=27.42, P<0.05), and significant positive correlation was found between ADC value and optical density (OD) value of AQP-4 (r=0.875, P<0.05). In the border region, cytotoxic edema began to appear at 6 h after HI injury and then vasogenic edema appeared and became predominant gradually. ADC value increased after a transient decrease and reached the peak at 48 h (F=50.25, P<0.05). The change of ADC value was concordant with the OD value of AQP-4, but no statistical correlation was found (r=0.505, P>0.05). The FA value decreased after HI. Conclusion DTI can be used to detect the brain edema in the early stage of HI injury. The change of ADC value correlates with that of AQP-4 expression.
