       目的 探讨全数字化乳腺摄影(FFDM)对乳腺不典型增生性病变的诊断价值。方法 回顾性分析87例临床扪诊阴性、仅以单纯钙化为表现的乳腺疾病患者全数字化乳腺摄影片,包括不典型增生28例(5例对侧伴发乳腺癌、3例病变局部伴多发乳头状瘤),其中轻度13例,中重度15例;乳腺增生症59例,其中3例病变局部伴多发乳头状瘤。放射科医生阅片均以BI-RADS为标准完成报告和分级,全部病例通过手术病理证实。结果 AH组致密型腺体15例,占53.57%,中间性钙化24例,占85.71%,单位面积钙化数量>5个/cm226例,占92.86%;而乳腺增生症组分别38例、50例和48例,占64.41%、84.75%和81.36%,两组之间的差异无统计学意义。钙化簇形态不规则在AH组19例,占67.86%,乳腺增生症组16例,占27.11%(P<0.05)。轻度与中重度不典型增生之间以上改变差异无统计学意义。结论 FFDM为以微钙化为惟一阳性发现的乳腺疾病提供了良好的诊断途径,并对乳腺不典型增生具有一定的诊断价值。
英文摘要:
      Objective To discuss the diagnositic value of full-field digital mammography (FFDM) for atypical hyperplasias (AH) of breast. Methods From March 2006 to October 2007, 87 patients with nonpalpable breast lesion and microcalcification were involved, including 28 patients of breast AH (5 cases with concomitant cancers in the contralateral breast and 3 cases with multiple papillomas) and 59 patients of proliferation of breast confirmed by surgery and pathology. The reports and grading for all cases were completed by two radiologists based on the standard of BI-RADS. Results Dense breast, intermediate microcalcification and microcalcification larger than 5/cm2 were found in 15, 24 and 26 patients in AH group respectively, whereas in 38, 50 and 48 patients respectively in the proliferation group. Irregular shape of the cluster was detected in 19 cases in AH group and 16 cases in proliferation group, the difference between the two groups was significant (P<0.05). Conclusion FFDM helps to detect nonpalpable breast lesion with microcalcification, and has important clinical value for atypical hyperplasias of breast.
