       目的 探讨应用实时三维超声心动图(RT-3DE)定量评价陈旧性心肌梗死患者左心室节段性收缩功能的可行性和临床价值。方法 应用RT-3DE测量28名正常人(对照组)和20例陈旧性心肌梗死患者(心肌梗死组)左心室壁17节段的舒张末期容积(rEDV)、收缩末期容积(rESV),计算节段每搏量(rSV)及节段射血分数(rEF)。 结果 对照组左心室壁17节段局部容积-时间曲线呈集中且规则的抛物线形,心肌梗死组呈运动分散且不规则的曲线;与对照组比较,心肌梗死组运动异常节段的rEDV、rESV显著增大(P<0.05),而REF显著减低(P<0.05);心肌梗死组17节段REF的平均值(rEF')与整体EF有良好的相关性(r=0.758,P<0.001)。结论 RT-3DE技术能够测定心肌梗死代偿节段的数目和分布范围,可用于定量评价左心室节段性收缩功能。
英文摘要:
      Objective To assess the feasibility and clinical value of real-time three-dimensional echocardiography (RT-3DE) in evaluating regional left ventricular systolic function in patients with old myocardial infarction (OMI). Methods Twenty-eight healthy volunteers and 20 patients with OMI were investigated, and the 17-segment myocardial model was adopted. The regional end diastolic volume (rEDV), regional end systolic volume (rESV), regional stroke volume (rSV) and regional ejection fraction (rEF)were assessed by RT-3DE. Results The 17-segment volume-time line was regular in healthy volunteers, but was irregular in OMI group. Compared with controls, rEDV and rESV of OMI group in anterior and inferior wall increased significantly, but rEF decreased significantly (P<0.05). Compensated segments were observed in patients with OMI. In the OMI group, rEF' was correlated with EF (r=0.758, P<0.001). Conclusion RT-3DE can reflect the number and scope of compensated segments. RT-3DE provides a new method in assessing the regional left ventricular systolic function.
