| 目的 观察超声检查对干燥综合征(SS)腮腺病变的诊断价值。 方法 对44例SS患者、19例可疑腮腺病变的口腔科患者(对照组1)、17名健康人(对照组2)进行双侧腮腺超声检查,根据声像图表现对受检者腮腺进行评价。 结果 SS组腮腺病变均累及双侧,实质声像图表现分为轻度不均匀、多发结节和纤维化萎缩三类。以多发结节和纤维化萎缩作为标准诊断,SS腮腺病变的诊断敏感性、特异性、阳性预测值和阴性预测值分别为84.09％、97.22%、97.37%和83.33%。SS组腮腺径线小于健康对照组,差异有统计学意义,其中以纤维化萎缩型腮腺径线最小。 结论 SS腮腺病变具有较特异的声像图改变;超声检查对诊断SS腮腺累及具有较高的诊断价值。 |

| Objective To evaluate the diagnostic efficiency of sonography in screening Sjgren's syndrome (SS) parotid gland involvement. Methods Forty-four SS patients (SS group), 19 patients with suspicious parotid gland diseases (control group 1) and 17 healthy subjects (control group 2) underwent parotid gland sonographic examination, and the sonographic features of the parotid gland were analyzed. Results Bilateral parenchymal changes were demonstrated in all SS patients. Parenchymal changes were divided into 3 types: slightly inhomogenous, multiple hypoechoic nodules and parenchymal atrophy. When the nodular and atrophy type were included in the diagnostic criteria, the diagnostic sensitivity, specificity, positive and negative predictive values was 84.09％,97.22%,97.37% and 83.33%, respectively. The size of parotid gland in the SS group (especially atrophy type) was significantly smaller than that of the healthy control group. Conclusion Advanced parenchymal changes demonstrated on ultrasound imaging almost invariably represent SS parotid gland involvement. Ultrasound is a useful tool for the diagnosis of parotid gland involvement in patients with SS. |
