目的 应用多普勒技术观察妊娠11+0~14+6周正常胎儿心脏大血管的血流动力学参数。方法 对230位正常妊娠孕妇在11+0~14+6周进行常规超声检查,同时观察胎儿心脏大血管。 结果 主动脉血流速度高于主肺动脉和动脉导管,与孕周之间呈正相关,大动脉的射血时间与孕周之间呈正相关,而射血加速时间与孕周之间呈负相关;妊娠11~14周胎儿静脉导管显示率相对较低,而静脉导管的血流速度、搏动指数、阻力指数与孕周之间无相关性;至妊娠13周以后主动脉、主肺动脉、动脉导管的流速、射血时间和射血加速时间以及静脉导管的血流参数相对恒定。 结论 妊娠13周以后可进行胎儿早期超声心动图检查,但胎儿心脏大血管的观察难度较大。胎儿心脏大血管的血流动力学表现可直接反映大血管本身状况,也可间接反应心脏的功能。当胎儿心脏结构或功能发生异常时,有可能引起大血管血流动力学的改变。 |

Objective To observe the hemodynamic status of cardiac large arteries in normal fetus between 11+0 to 14+6 weeks' gestation with Doppler ultrasound. Methods There were 230 pregnant women who underwent a routine examination by transabdominal ultrasonography between 11+0 to 14+6 gestational weeks, and the hemodynamic parameters of cardiac large arteries were observed. Results The blood flow velocity of aorta (AO) was higher than that of pulmonary artery (PA) and ductus arteriosus (DA), and the velocity of these large arteries had a positive correlation with the weeks' gestation. The eject time (ET) of arteries had a positive correlation with the gestational weeks, but the accelerated time (AT) of ejection in arteries had a negative correlation with the gestational weeks. The ductus venous (DV) had a poor visualization in 11―14 weeks' gestation, and the blood velocity, pulse index (PI) and resistant index (RI) had no correlation with the gestational weeks. The blood flow velocity of AO, PA, DA and DV, ET and AT were relatively constant after 13 weeks' gestation. Conclusion The fetal echocardiography examination can be used after 13 weeks' gestation, but the fetal cardiac large arteries can be observed hardly. The hemodynamics findings of cardiac large arteries can reflect the condition of these arteries directly, and reflect the function of heart indirectly. The hemodynamics characteristics of large arteries can be changed when the structure or function of fetal heart develops abnormal. |
