      目的 探讨乳腺癌MR动态增强扫描(DEC)最大密度投影(MIP)重建图像所示肿瘤血管形态特征与肿瘤大小、伴随淋巴结肿大、时间-信号强度曲线(TIC)类型及肿瘤病理分级间是否存在相关性。 方法 对48例细针穿刺活检证实为乳腺癌的患者治疗前均行动态增强MR扫描。增强早期(第一时相)图像减影后行3D-MIP重建,根据重建图像所示肿瘤血管的特征分级(正常血管,异常动脉和肿瘤血管)。在AW4.2工作站上测量肿瘤最大直径、观察双侧腋窝淋巴结情况、使用Functool软件绘制动态TIC(渐进型、平台型、流出型)。对3D-MIP所示肿瘤血管形态分级与肿瘤直径、淋巴结转移情况、TIC类型及肿瘤病理分级进行Spearman等级相关分析及Kappa相关性检验。 结果 3D-MIP所示肿瘤血管形态分级与腋窝淋巴结及肿瘤病理分级呈正相关(P<0.05),与动态增强扫描TIC类型间具有较强相关性(r=0.840)。两者经Kappa检验,Kappa=0.761,一致性较好。 结论 乳腺癌MR动态增强扫描3D-MIP示肿瘤血管形态与TIC类型间存在较强相关性,可以通过3D-MIP所示的肿瘤血管特征推测TIC类型。

      Objective To investigate the correlation between tumor vessel morphology showed by three-dimension maximum intensity projection (3D-MIP) and the clinical and pathological characteristics of breast cancer, such as lymph node metastasis, time-signal intensity curve (TIC) and pathological grade. Methods Forty-eight patients with breast cancer confirmed by needle biopsy underwent breast dynamic contrast enhanced MRI (DCE-MRI) examination on 1.5 T MR scanner. On AW4.2 workstation, 3D-MIP reformation were performed using subtracted images, and tumor vessels were divided into three grades by features of modality. Tumor size, lymph node, type of TIC and histologic grade were compared with the degree of tumor vessel (3D-MIP) by Spearman correlation and Kappa analysis. Results There was positive correlation of tumor vessel morphologic feature with lymph node and tumor pathological grade (P<0.05), and there existed good correlation between type of TIC and tumor vessel morphologic feature showed by 3D-MIP (r=0.840, Kappa=0.761, P=0.000). Conclusion The correlativity between type of TIC and tumor vessel feature showed by 3D-MIP of breast cancer by DCE-MRI is good. It is possible to infer the type of TIC of breast cancer by 3D-MIP.
