       目的 探讨肺动脉悬带的超声心动图特征,评价右侧胸骨旁透声窗对该畸形的诊断价值。方法 由超声心动图检查提示、并经螺旋或多排CT及手术证实的肺动脉悬带患儿4例,对比分析其各切面超声心动图表现特征,探讨右侧透声窗对该畸形的诊断价值。结果 超声心动图多切面均显示正常肺动脉分叉处左肺动脉缺失;右侧胸骨旁切面清晰显示迷走左肺动脉起始部和整个中、远段走行4例;剑下短轴切面清晰显示左肺动脉起始部3例,1例模糊;左侧胸骨旁短轴切面清晰显示左肺动脉起始部2例,模糊2例;胸骨上窝短轴切面仅显示左肺动脉起始部2例,较模糊。结论 右侧胸骨旁透声窗对提高超声心动图诊断肺动脉悬带的准确率有重要临床价值。

      Objective To explore the echocardiographic characteristics and evaluate the significance of right parasternal approach in diagnosing pulmonary artery sling. Methods Echocardiographic data of 4 cases with pulmonary artery sling, confirmed by spiral or multi-slice CT and surgical operation, were analyzed and echocardiographic characteristics of each plane was compared to evaluate the role of right parasternal views in the diagnosis of this anomaly. Results Left pulmonary artery was absent at normal bifurcation site on multiple planes in all 4 cases; the right high parasternal short axis view clearly demonstrated the origin and initial posterior, middle segment and leftward course of the left pulmonary artery in all 4 cases; the right oblique subxiphoid views clearly demonstrated origin of left pulmonary artery in 3 cases, vaguely in 1 case; the left parasternal short axis planes clearly showed origin of the left pulmonary artery in only 2 cases, vaguely in 2 cases; the suprasternal short axis views vaguely revealed the origin of left pulmonary artery in only 2 cases. Conclusion The cross sectional echocardiography and color Doppler imaging can offer a definite diagnosis in infants with pulmonary artery sling. The right parasternal approach plays a very important role in the diagnosis of this anomaly.
