       目的 应用实时三维超声心动图(RT-3DE)技术观察继发孔型房间隔缺损(S-ASD)面积的大小及其在心动周期各时相的动态变化特征,并评价房间隔缺损的面积变化率及其影响因素。方法 应用RT-3DE技术对30例S-ASD患者进行实时三维成像,测量心动周期中各帧S-ASD的面积,并由此计算出各帧面积所占最大面积的百分比及S-ASD面积变化率,并分析S-ASD面积变化率与年龄、心率、分流量(Qp/Qs)、平均面积之间有无相关性。结果 在心动周期各时相中,S- ASD面积收缩末期最大(6.37±1.17)cm2, 舒张末期最小(3.17±0.68)cm2;等容舒张期、等容收缩期、舒张晚期,S-ASD面积的变化较小;舒张中期及收缩中、晚期,S-ASD面积的变化较大。S-ASD面积的变化率与年龄呈低度负相关(r=-0.577, P<0.05),与Qp/Qs、心率、平均面积无显著相关性。结论 S-ASD的面积在心动周期各时相中有典型的动态变化特征,RT-3DE可直观地显示并动态地探测这种变化特征。

      Objective To observe the dynamic characteristics of secundum atrial septal defect (S-ASD) area during cardiac phases and to investigate factors which influence S-ASD dynamics by real-time three-dimensional echocardiography (RT-3DE). Methods Thirty patients with S-ASD were examined using RT-3DE. Each defect area during cardiac phases was measured, and the ratio between defect area and maximum area was calculated. The area change percentage of S-ASD was analyzed, as well as its correlation with age, heart rate, pulmonary to systemic flow (Qp/Qs) and the mean area of S-ASD. Results The defect area was the largest in end systole (6.37±1.17)cm2, while the smallest in end diastole (3.17±0.68)cm2. The defect area changed slightly during isovolumetric diastole, isovolumetric systole and end diastole, but changed obviously during mid-systole, mid-diastole and end systole. There was slight inverse correlation between the area change percentage and the age of patients (r=-0.577, P<0.05), but the mean area of S-ASD, the Qp/Qs and the heart rate showed no significant influence on the area change percentage. Conclusion RT-3DE can show the dynamic characteristics of S-ASD during cardiac phases directly and dynamically.
