目的 采用超声斑点追踪成像(STI)技术对猪闭合性心肌挫伤前、后对左心室短轴各节段不同形式的运动进行评价。方法 选取健康贵州小型巴马猪9只,采用小型撞击器进行撞击, 成功建立闭合性心肌损伤的动物模型后,应用二维应变技术分别于撞伤前正常状态及撞伤后0.5 h、2 h、4 h、8 h及12 h行超声心动图检查,采集左心室短轴二尖瓣水平、乳头肌水平及心尖水平心室节段二维图像(帧频>40帧/s),应用超声斑点追踪成像技术分析软件分析撞击前后左心室心肌各节段的径向应变(RS)、圆周应变(CS)及旋转角度(Rot)。根据病理结果将左心室各节段分为损伤区及非损伤区,分析RS、CS及Rot变化及与LVEF的相关性。结果 撞伤前正常状态下显示各节段RS、CS、Rot差异无统计学意义(P>0.05)。撞伤后STI分析显示撞伤后损伤区RS及Rot撞伤后0.5 h即呈逐渐下降趋势,较撞伤前显著下降,差异有统计学意义(P<0.05, P<0.01)。损伤区CS在撞伤后4 h出现降低 (P<0.05, P<0.01)。结论 STI能敏感地检测出闭合性心肌损伤的动物模型的局部心肌收缩功能的变化。 |

Objective To estimate the movement at different segments of the short axis of the left ventricle before and after close myocardial contusion of pigs with speckle tracking imaging (STI) technique. Methods Nine small Guizhou-Panama pigs were enrolled. The close myocardial contusion animal model was successfully built with a small impactor. STI was applied before and 0.5 h, 2 h, 4 h, 8 h and 12 h after the injury, respectively, and 2DE images (frequency>40 frame/s) of left ventricle at mitral valve, papillary muscle and apical levels were analyzed by STI technique. STI analysis software was used to get the left ventricle RS, CS and Rot at different levels before and after the MC, also the degree of the LVEF decrease was observed. The changes of RS, CS and Rot and the relativity to LVEF were analyzed. Results There was no significant difference for the RS, CS and Rot of the levels mentioned above on left ventricle before the MC under normal state (P>0.05). STI analysis software showed that RS and Rot of trauma area gradually decreased 0.5 h after the strike and significantly decreased compared with pre-myocardial contusion (P<0.05, P<0.01). CS of trauma area gradually decreased 4 h after the strike and significantly decreased compared with pre-myocardial contusion (P<0.05, P<0.01). Conclusion STI enables sensitively detection of the regional contraction changes. |
