      目的 探讨原发性高血压及高血压脉压差增大患者血管内皮舒张功能的改变与左室舒张期二尖瓣M型EF斜率改变间的关系。 方法 采用彩色多普勒高频超声对45例原发性高血压患者(A组),45例高血压脉压差增大患者(B组)及45例正常血压(C组)的血管内皮舒张功能和左室舒张期二尖瓣M型EF斜率等进行检测并对每组含服硝酸甘油后肱动脉内径和血流的变化进行对比分析。 结果 本组病例A组+B组与C组比较,收缩压(SBP)、舒张压(DBP)均明显升高,统计学处理有非常显著差异(

      Objective To discuss the relation between the changes of BVE diastolic function of the patients with the primary hypertension and hypertension pulse pressure amplification and the changes of EF slope of left ventricular mitral valve type-M in period of relaxation. Methods Using color Doppler high frequency ultrasound, examine BVE diastolic function and EF slope of left ventricular mitral valve type-M of 45patients of the primary hypertension (Group A), 45 patients of hypertension pulse pressure amplification (Group B) and 45 persons with normal pressure (Group C) and make comparison and analysis of the changes of the blood flow and bore of the brachial artery after taking glyceryl trinitrate. Results Comparing patients of Group A + Group B with Group C, SBP and DBP are increasing obviously. There is distinct difference in statistics (P<0.01). And there is also obvious difference between Group A of the primary hypertension and Group B hypertension pulse pressure amplification in statistics (P<0.01). There is no significant meaning in comparing differences of BD index of bore of the brachial artery among different groups (P>0.05)in statistics. FMD index caused by reactivity congestion in Group A and Group B has obvious attenuation. Compared with Group C, it has significant difference(P<0.01). Comparing Group A with Group B, the index has statistic meaning (P<0.05). Increased PP amplification and FMD attenuation have obvious negative correlation (r=-0.89, P<0.01). FMD weakening has obvious correlation with decrease of EF slope of mitral valve (r=0.90, P<0.05). No differences appear in comparing blood vessel disatolization caused by glyceryl trinitrate in the three different groups with EF slope (P>0.05). Conclusion Patients with the primary hypertension have BVE diastolic dysfunction. And BVE diastolic dysfunction is strengthened with increased PP amplification. Attenuation of BVE diastolic function of patients with the primary hypertension has close relation with reduction of BEV. Color Doppler high frequency ultrasound is a simple, economic, atraumatic and reliable method to examine BVE diastolization.
