目的 探讨彩色多普勒超声心动图诊断小儿左冠状动脉异常起源于肺动脉 (ALCAPA)的价值。方法 收集 11例ALCAPA患儿资料,回顾性分析经胸超声心动图表现,并与心血管造影和手术结果相对比。结果 10例ALCAPA超声心动图表现:二维显示左冠状动脉在主动脉根部缺如、右冠状动脉扩张、左心室乳头肌回声增强;彩色多普勒显示左冠状动脉与肺动脉连接的异常血流信号,8例患儿有丰富的侧支血管形成。结论 超声心动图检查具有无创、实时显像、动态观察等优点,可早期诊断ALCAPA。 |

Objective To assess the value of color Doppler flow mapping in diagnosing anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA). Methods Eleven cases of hospitalized ALCAPA were reviewed retrospectively. The echocardiographic features were analyzed in all patients compared with cardiac catheterization and surgical founding. Results There were 10 ALCAPA in 11 patients. The characteristics of 2D echocardiogram findings included absent left coronary artery ostium from aortic root, dilated right coronary artery and increased echogenicity of the papillary muscles. Visualization of color-flow Doppler detected abnormal flow on the conection of left coronary artery and the pulmonary trunk. Collateral vessels had rich blood flow in eight patients. Conclusion Echocardiography is a non-invasive, real-time image and dynamic observation method for diagnosis of ALCAPA. |
