目的 应用MRI评价两种在体人工心脏瓣膜Medtronic-Hall(M-H)单叶和Sorin双叶机械人工心脏瓣膜功能。方法 对7例置换双叶机械瓣、6例单叶机械瓣的13例人工主动脉瓣膜置换患者进行MR血流速度测量,于升主动脉内距主动脉瓣环为人工瓣环直径的0.5、1.0及2.0(0.5 D、1.0 D、2.0 D)处进行血流速度测量,应用血流分析软件得到平均前向流量、平均反向流量、平均净前向流量、单位体表面积平均净前向流量、平均速度、峰值速度、平均净前向流量面积,采用MATLAB 6.5软件进行2 D及3 D血流图形重建。结果 M-H单叶瓣组和Sorin双叶瓣组的血流参数间差异无显著统计学意义,但MR血流速度剖面图两组显示不同:M-H瓣组3个层面在一个心动周期内呈现右手螺旋血流模式,而Sorin瓣组仅2.0 D层面有右手螺旋的趋势。结论 M-H单叶瓣和Sorin双叶瓣都能保证生理需要的血流量,但M-H单叶瓣的血流模式可能更接近生理血流。 |

Objective To observe blood dynamic after Medtronic-Hall (M-H) valve and Sorin Bicarbon aortic heart valve implantation in vivo with MRI. Methods Blood velocities were measured in 13 cases with prosthetic aortic valve, including 6 cases with M-H valve and 7 cases with Sorin Bicarbon aortic heart valve at three positions near the valve in the half, one and two diameter downstream in the ascending aorta with MRI. The reverse volume, net forward volume, net forward volume/BSA, forward velocity, peak velocity and forward area were analyzed qualitatively and compared between two groups using flow analysis software, respectively. Two-dimensional and three-dimensional surface plots of velocity were obtained using MATLAB 6.5 software field displayed. Results No statistical difference was found between M-H group and Sorin group in all parameters of all three positions. Three-dimensional and two-dimensional surface profiles in downstream of the prosthetic aortic valve showed right-handed helical forward flow in all three positions during a cardiac cycle in M-H group, but only in 2.0 D position in Sorin group. Conclusion Both M-H valve and Sorin valve could maintain the physiological blood volume in a cardiac cycle after implantation. The blood flow downstream of M-H valve is more similar to physiological flow than that of Sorin valve. |
