       目的 评价单侧放射冠缺血性脑卒中患者运动相关的大脑半球激活部位和偏侧化指数及其与Fugl-Meyer评分之间的相关性。方法 选取5例正常志愿者与12例单侧放射冠缺血性脑卒中患者进行血氧水平依赖功能磁共振成像(BOLD-fMRI)检查。试验采用Block设计,运动任务为顺序对指任务。采用SPM2进行数据分析和脑功能区定位,计算不同感兴趣区激活体素数目及偏侧化指数(LI)。结果 梗死部位位于单侧放射冠的患者脑部激活范围较对照组广泛,表现为双侧运动传导通路的激活;左侧颞叶的激活出现在所有卒中组,而未出现在对照组。双侧大脑半球、初级感觉运动区(SMC)、第一躯体运动区(M1区)的LI在对照组与卒中组之间的差异有统计学意义(P=0.002;P=0.002;P=0.008);在卒中组,LI(半球、SMC、M1)与患者上肢Fugl-Meyer评分之间的相关性差异无统计学意义(r=0.133,P=0.618;r=0.558,P=0.059;r=0.297,P=0.348)。结论 单侧放射冠脑梗死后,与运动任务相关的脑区激活范围存在明显偏侧化现象;偏侧化程度与患者上肢运动功能状况无显著相关性;左侧颞叶的激活可能在脑卒中患者早期运动执行过程中起一定作用。

      Objective To evaluate the relationship between motor-related activated cortices and Fugl-Meyer score in patients with corona radiata infarction. Methods Twelve patients (8 females and 4 males, 38-71 years old) with solitary lacunar infarction of unilateral corona radiata (5, right side; 7, left) and 5 healthy subjects were recruited in the study, each of whom underwent blood-oxygen level dependent functional MRI (BOLD-fMRI) examination by a 3.0T MR scanner. The fMRI was designed as a block task of affected hand-movement. The data were analyzed by SPM2. The activated voxels and laterality index (LI) were calculated and compared between patients and controls. Correlative analysis was performed between LI and the Fugl-Meyer score in patients. Results The activated cortices in patients were larger than that of controls. The LI of bilateral hemisphere, sensory motor cortex (SMC) and M1 were higher in controls (P=0.002; P=0.002; P=0.008). No statistical correlation was found between LI (hemisphere, SMC, M1) and Fugl-Meyer score (r=0.133, P=0.618; r=0.558, P=0.059; r=0.297, P=0.348). The activated left temporal lobe was present in patients but absent in controls. Conclusion Obvious lateralization related to motor task is revealed in patients with corona radiata infarction. Transversely, it is not parallel to the functional status of affected upper extremities. The activation of left temporal lobe may play a role in the motor function recovery in those patients.
