       目的 探讨大鼠心搏骤停和复苏后脑缺血损伤的MR表现及其病理基础。方法 SD大鼠20只,分为心搏骤停复苏组和对照组。前者应用心搏骤停和复苏机械装置,行心室致颤和复苏,后者行手术操作,不致颤和复苏。复苏组在复苏前及后,对照组在术前及术后1 d、3 d、7 d、14 d,行MR T1WI及T2WI检查,观察脑组织信号强度变化,并测量脑组织与肌肉信号强度比及脑组织病理观察。结果 复苏组MR表现为双侧海马及颞叶局灶性异常信号区,而对照组未见异常信号。1~7 d复苏组海马缺血损伤T1WI及T2WI信号强度比与对照组差异有统计学意义(P=0.00~0.02、0.00~0.04<0.05),其改变在1~3 d达高峰,14 d逐渐恢复正常。颞叶缺血损伤T1WI信号强度比在1~3 d、T2WI信号强度比在1~7 d与对照组差异有统计学意义(P=0.00~0.01、0.00~0.03<0.05),其改变在1~3 d达高峰,7 d逐渐恢复正常。脑缺血损伤后24 h海马CA1、CA3、DG区和颞叶皮层等部位即出现死亡细胞,7 d时达高峰。DG区死亡细胞相对于海马其他区及颞叶皮层较少,而海马CA1区则相对其他区为最多。结论 大鼠脑复苏后颞叶及海马发生缺血损伤,海马部位较颞叶持续时间长,MR可动态反映复苏后大鼠脑缺血损伤的病理变化过程和损伤程度,可作为心搏骤停和复苏后全脑缺血损伤评价的有效手段。

      Objective To investigate MR findings and pathological basis of global cerebral ischemic injury after cardiac arrest and CPR by using rat models of resuscitation. Methods Twenty Sprague-Dawley (SD) rats were randomly selected and divided into resuscitation group (n=14) and control group (n=6). In resuscitation group, the model was established using a set of customized mechanic instrument to produce cardiac arrest and then CPR. In control group, the rats only suffered from necessarily same surgical procedures without cardiac arrest and CPR. MR as well as brain histological examination was performed before and 1 d, 3 d, 7 d, 14 d after the production of model. Results At MR imaging, patchy abnormal signal intensity was found on T1WI and T2WI on the bilateral hippocampus as well as temporal cortex in resuscitation group, while no signal intensity changes in control group. There was statistical difference found in the hippocampus-muscle signal intensity ratio between hippocampus of the resuscitation and control group at the time points from 1 to 7 d (P=0.00-0.02; 0.00-0.04).The signal intensity ratio of hippocampus peaked at 1 to 3 d and returned nearly normal level at 14 d. There was statistically difference of temporal-muscle signal intensity ratio of T1WI and T2WI between the resuscitation and control group in 1-3 d and 1-7 d, respectively (P=0.00-0.01 and 0.00-0.03).The signal intensity ratio of temporal cortex reached its peak at time points from 1 to 3 d and normalized at 7 d. Pathologically, in resuscitation group, lots of dead cells primarily in CA1, CA3, DG portion of hippocampus as well as temporal cortex were found since 24 h of the global cerebral ischemic injury. The dead cells increased gradually and peaked in 7 d. The amount of dead cells in DG zone of hippocampus was less than that of the other zones of hippocampus as well as temporal cortex. Conclusion The global cerebral ischemic injury of the hippocampus as well as temporal cortex is relatively severer than that of temporal cortex after the cardiac arrest and CPR in rat models. The dynamic change of T1 and T2 signal intensity can reflect the pathological change and the severity of injury.
