       目的 评价质子磁共振频谱各参数相对值在急性一氧化碳中毒后迟发性脑病(DNS)诊断中的应用价值。方法 临床拟诊的急性一氧化碳中毒后迟发性脑病组34例,正常对照组40例,分别进行常规MR和质子磁共振波谱(1H-MRS)检查,测量各感兴趣区化合物的相对值。结果 获取的患者波谱数据显示NAA/Cr(0.57±0.08), Cho/Cr(2.40±0.08), β, γGlx/Cr (2.17±0.07)。同对照组比较β,γGlx/Cr及Cho/Cr升高,但NAA/Cr降低。DNS组与正常对照组NAA/Cr, Cho/Cr, β, γGlx/Cr差异有统计学意义(P<0.05),同时发现在0.9~1.33 ppm间Lip峰值升高,峰下面积增加。结论 1H-MRS能检测出DNS患者脑部病灶的代谢异常,为常规MR诊断提供有益的补充信息。

      Objective To evaluate the role of proton magnetic resonance spectroscopy (1H-MRS) in the diagnosis of delayed neuropsychological sequelae after carbon monoxide poisoning (DNS ). Methods MR and 1H-MRS were performed in 34 DNS patients and 40 healthy volunteers, and the ratio of the peak of each organic compound was measured in each volume of interest. Results The data spectra obtained from DNS patients showed NAA/Cr (0.57±0.08),Cho/Cr (2.40±0.08), β, γGlx /Cr (2.17±0.07). Compared with the control group,the β,γGlx /Cr ratio and Cho/Cr ratio increased, but the NAA/Cr ratio decreased. Significant difference existed between DNS and control group on the ratio of NAA/Cr, Cho/Cr,β, γGlx /Cr (P<0.05). In spectroscopy, the Lip evaluated peak in 0.9-1.33 ppm was detected. Conclusion 1H-MRS can detect metabolic abnormalities in brain lesions from DNS patients and provide complementary information for conventional MR.
