| 目的 探讨超声引导下注入不同药物治疗盆腔囊性病变的方法及疗效。 方法 超声引导下介入治疗301例4组323个盆腔囊性病变,A组为120例130个卵巢浆液性囊肿,B组为93例盆腔包裹积液,C组为79例91个卵巢巧克力囊肿,D组为9例卵巢黏液性囊肿,并按注入药物不同将A组和B组各分为3个亚组,Ⅰ亚组单纯注入2-4组抗感染三联药,Ⅱ亚组单纯注入相当囊内液1/3的无水乙醇,Ⅲ亚组注入相当囊内液1/4的无水乙醇和2-4组抗感染三联药;C组和D组均注入相当于囊内液1/2的无水乙醇(最高治疗量200 ml),分别观察比较治疗后3、6个月A、B组的各亚组之间的疗效及A、B组的Ⅱ亚组与C组、D组之间的疗效。 结果 A、B组中的Ⅲ亚组疗效最佳,治疗后6个月治愈率分别为95%(41/43)、94%(29/31),Ⅱ亚组的治愈率分别为91%(40/44)、90%(28/31),有效率均为100%,均明显高于Ⅰ亚组,与Ⅰ亚组之间比较差异有统计学意义(P＜0.05);单纯注入无水乙醇的4组囊性病变的总治愈率和总有效率分别为87%(153/175)、99%(173/175),各组疗效均高于D组,与D组之间比较差异有统计学意义(P＜0.05)。 结论 超声引导下注入不同药物介入治疗盆腔囊性病变疗效显著。 |

| Objective To investigate the effect of ultrasound guided interventional therapy in different pelvic cysts. Methods The ultrasound guided interventional therapy was performed on 301 patients with 323 pelvic cysts, including 120 cases with 130 ovarian serosity cysts (group A), 93 encapsulated pelvic effusions (group B), 79 cases with 91 endometrial cysts (group C) and 9 ovarian mucous cysts (group D). Patients in group A and B were separated into three subgroups, and patients in subgroupⅠ,Ⅱ, Ⅲ were treated with three kinds antiphlogiston combination, injection of 99.7% ethanol (1/3 volume of the cyst) and combinated antiphlogiston and ethanol (1/4 volume of the cyst), respectively. Patients in group C and D were treated with injection of 99.7% ethanol (1/2 volume of the cyst, maximum dose was 200 ml). All patients were followed up 3 months and 6 months post operative. Results The cure rate of subgroup Ⅲof group A and B was 95% (41/43) and 94% (29/31), while of subgroup Ⅱ was 91% (40/44) and 90% (28/31), respectively, being significant different from those of subgroup Ⅰ (P＜0.05). The total cure rate and effective power of different pelvic cysts treated with injection of 99.7% ethanol was 87% (153/175) and 99% (173/175), significant different from that of group D (P＜0.05). Conclusion The effect of ultrasound guided interventional therapy of pelvic cysts is noticeable. |
