| 目的 探讨核素淋巴显像诊断四肢淋巴水肿的价值及其影像特征。 方法 对117例四肢水肿(94例下肢水肿和23例上肢水肿)患者行SPECT淋巴显像,分别获得5 min,15 min,30 min或延迟1~2 h影像,对图像进行定性分析,并与术后或临床最终诊断结果比较。 结果 117例四肢水肿中临床确诊淋巴水肿108例;核素显像94例下肢水肿,诊断为淋巴性水肿78例(包括原发或继发),淋巴回流正常6例,淋巴回流缓慢10例;23例上肢水肿(全部有乳腺癌手术史)包括淋巴性水肿22例,淋巴回流正常1例。核素淋巴显像诊断四肢淋巴水肿的灵敏度及特异性分别为92.6%及100%。 结论 核素淋巴显像简便无创、安全可靠,能鉴别四肢水肿性质,对确定治疗方案和手术患者的选择有独特价值。 |

| Objective To assess the diagnostic value of radionuclide lymphoscintigraphy in extremity lymphedema. Methods Lymphoscintigraphy was performed with SPECT on 117 cases with history of extremity lymphedema, including 94 lower extremity lymphedema and 23 upper extremity lymphedema, and the imagings were analyzed comparing with post-operation findings. Results Lymphedema was diagosed in 78 of 94 lower extremity, also in 22 of 23 upper extremity cases. The sensitivity and specificity of radionuclide lymphoscintigraphy was 92.6% and 100%, respectively. Conclusion Radionuclide lymphoscintigraphy is of great value in the diagnosis and the choice of treatment for extremity lymphedema. |
