| 目的 探讨64层螺旋CT多期动态增强扫描对临床常见的腮腺淋巴瘤、多形性腺瘤及恶性肿瘤诊断和鉴别诊断的价值。 方法 对经病理证实的30例腮腺肿瘤共32个病灶进行回顾性研究,所有病例于术前均行Siemens Sensation 64层螺旋CT平扫及增强检查,分析评价其CT多期动态增强表现特征(强化方式及强化程度)。 结果 在32个病灶中包括腺淋巴瘤5例(7个病灶),多形性腺瘤14例,恶性肿瘤11例。腺淋巴瘤表现为典型的"快进快出";多形性腺瘤表现为特征性的渐进性强化;恶性肿瘤在增强早期快速强化,强化峰值在静脉期(90 s);三者中腺淋巴瘤强化程度最高。 结论 64层螺旋CT多期动态增强扫描能为腮腺肿瘤的定性诊断提供较多依据。 |

| Objective To assess the value of 64-slice dynamic contrast-enhanced CT in diagnosis of common parotid tumors. Methods A retrospective study was performed on 30 cases suffering from parotid tumors who underwent Siemens Sensation 64-slice dynamic contrast-enhanced CT, including 32 histopathologically proved tumors. CT features, like the mode and the extent of intensification were analyzed and evaluated. Results There were 5 cases with Warthin tumors (7 tumors), 14 cases with pleomorphic adenomas (14 tumors) and 11 parotid malignancies (11 tumors). Warthin tumors showed a typical intensification of contrast "quickly-in and quickly-out", reached the peak time of enhancement at arterial phase. A gradually increasing intensification of contrast was showed typically on pleomorphic adenomas. An early intensification of contrast was observed in the malignant tumors, but peak time was at venous phase (90 s). Moreover, warthin tumors surpassed the others in the extent of intensification. Conclusion 64-slice dynamic contrast-enhanced CT can provide much evidence for the histological diagnosis of parotid tumors. |
