| 目的 探讨乳腺黏液癌的声像图特征,提高超声诊断的准确率。 方法 回顾分析17例乳腺黏液癌二维及彩色多普勒声像图表现,探讨声像图与相应的病理类型之间的关系。 结果 17例乳腺黏液癌中术前超声诊为断恶性肿瘤12例,可疑恶性5例,术后病理诊断单纯型黏液癌13例(76.5%),腋窝淋巴结转移1例;混合型黏液癌4例(23.5%),腋窝淋巴结转移2例。单纯型黏液癌肿块横径>纵径,CDFI显示肿瘤周边血流呈放射状分布,内部偶见星点状血流信号;混合型黏液癌纵径>横径,CDFI显示肿瘤周边血流信号稀少,内部血流较丰富。两种类型黏液癌形态不规则,CDFI显示血流阻力指数较高(RI>0.80)。 结论 不同病理类型的乳腺黏液癌声像图具有一定特异性,认识这些特征有助于乳腺癌的鉴别诊断,对临床选择治疗方案具有重要的指导意义。 |

| Objective To investigate the sonograghic features of the breast mucinous carcinoma and improve the accuracy of ultrasound diagnosis. Methods The 2-dimensional (2D) sonogram and color Doppler flow imaging (CDFI) were used to retrospectively analyze 17 breast mucinous carcinomas, thus to discuss the relation between ultrasonography and pathology. Results Pre-operational ultrasound diagnosis of 12 cases were malignant, 5 cases were dubiously malignant.Post-operational pathology confirmed 13 cases were simple mucinous carcinoma (76.5%) with axillary metastatic lymph nodes in one case; 4 cases were mixed mucinous carcinoma (23.5%) with axillary metastatic lymph nodes in two cases. In simple mucinous carcinomas, the transvers/anteroposterior diameter ratio>1, CDFI showed actinomorphous blood signals on borders and occasional star point flow in the foci. In mixed mucinous carcinomas, the transvers/anteroposterior diameter ratio<1, rare blood signals on the border and abundant blood flow in the foci were seen in CDFI. The shapes were irregular in both carcinomas and CDFI showed high resistance index (RI>0.80). Conclusion The sonographic imagings are characteristic in different pathologic types of breast mucinous carcinoma. To know the specificities is helpful to the differential diagnosis of breast carcinoma and also a guidance to choose therapeutic regimen in clinic. |
