目的 应用超声斑点追踪成像技术(STI)检测糖尿病患者左心室收缩功能,探讨STI评价左心室长轴局部及整体收缩功能的价值。方法 2型糖尿病患者66例,分为单纯糖尿病组41例,糖尿病并左心室肥厚组25例;正常对照组37例。获取收缩期左心室长轴基底段、中段、心尖段峰值应变,并计算左心室基底段、中段、心尖段收缩期平均峰值应变及左心室整体应变值。所有受检者的左心室射血分数值正常。结果 两组糖尿病患者左心室长轴室壁各节段收缩期纵向峰值应变均明显低于对照组(P<0.01),糖尿病并左心室肥厚组除心尖段外,其他各节段峰值应变低于单纯糖尿病组(P<0.05);两组糖尿病患者左心室长轴基底段、中段、心尖段收缩期平均峰值应变及左心室整体应变明显低于对照组(P<0.001),而糖尿病并左心室肥厚组较单纯糖尿病组降低(P<0.01)。结论 超声斑点追踪成像技术能够评价2型糖尿病患者早期左心室长轴局部和整体收缩功能异常。 |

Objective This study investigated the long-axis global and regional systolic function of the left ventricle (LV) in patients with type 2 diabetes mellitus (T2DM) using two-dimensional speckle tracking imaging (STI). Methods STI was performed in 103 subjects with normal ejection fraction: 41 with diabetes mellitus only (DM group), 25 with both diabetes and left ventricular hypertrophy (DH group), and 37 normal controls. Longitudinal peak systolic strain of six walls in apical long-axis, four-chamber, and two-chamber views was measured, and average peak strain in the long axis at basal, middle and apical levels, and LV global peak strain were assessed. Results Peak strain of each plane in all patients (DM, DH) was significantly reduced when compared with controls(P<0.01), and peak strain of basal and middle segments in DH group was lower than in DM group (P<0.05). All patients showed significantly reduced average peak strain in the long axis at basal, middle and apical levels, and global peak strain compared with controls (P<0.001), and DH group was significantly lower than DM group (P<0.01). Conclusion STI may be used to identify early abnormalities in patients with T2DM who have normal left ventricular systolic function. |
