目的 应用斑点追踪显像技术(STI)评价射血分数(EF)正常的肥厚性心肌病(HCM)心肌应变及收缩同步性的变化情况,并探讨左室流出道梗阻对于上述指标的影响。方法 HCM组(n=34)和正常对照组(n=30)分别行超声心动图检查,根据静息状态下左心室流出道压差将HCM组分为梗阻型(n=15)和非梗阻型(n=19)。采用STI技术测量患者径向(RS)、圆周方向(CS)、纵向(LS)、横向应变(TS)幅度以及左心室短轴乳头肌水平室间隔、左室后壁到达径向峰值应变的时间差值(SPWPSD)。结果 HCM组与正常组相比EF值无明显差异,但各种应变均显著降低,SPWPSD升高。分组资料表明梗阻型患者较非梗阻型应变进一步降低,不同步性高于非梗阻型患者。结论 HCM早期即可出现心肌变形能力下降及收缩不同步,左心室流出道梗阻加剧上述变化。 |

Objective Speckle tracking imaging (STI) has been proposed as a novel tool to quantify myocardium function and to detect early systolic abnormality. However, the application in asymmetric hypertrophic cardiomyopathy (HCM) with preserved ejection fraction (EF) is still limited, and the different components of strain and systolic dyssynchrony derived from radial strain between obstruction type and non-obstruction type have not yet been explored. The aim of the study was to assess the alteration of the regional myocardium function as well as systolic dyssynchrony in HCM with STI. Methods Echocardiography was performed in 34 HCM patients (normal EF; 15 with obstruction, 19 without obstruction at resting) and 30 normal controls. Radial strain (RS), circumferential stain (CS), longitudinal strain (LS) and transverse strain (TS) were analyzed with STI in left ventricle (LV) short-axis and apical views. LV systolic dyssynchrony was also calculated as the time difference between septal and posterior wall peak strain (SPWPSD) at the level of short-axis papillary muscle view. Results Compared with healthy controls, all components of strain reduced in HCM with increase of systolic dyssynchrony. Further analysis showed more deterioration occurred in patients with obstruction than those without obstruction. Conclusion Deformation abnormality could deteriorate in preserved LVEF HCM which can be detected by STI. Increasing afterload may induce reduction of myocardium deformation as well increase of systolic dyssynchrony. |
