      目的 应用vQCT技术测量绝经后女性股骨颈的容积性骨密度(BMD)和几何参数,评价各参数对骨质疏松性骨折的预测能力。方法 选取绝经后妇女47例,其中正常组26例,骨质疏松组14例,骨质疏松伴腰椎骨折组7例。应用多层螺旋CT对三组患者左股骨颈进行容积扫描并薄层重建,利用Osteo CAD软件对多平面重组图像进行测量,计算出股骨颈的皮质骨、小梁骨和整体骨的容积BMD,股骨颈轴长和最小横截面积,并进行统计学分析。结果 正常组与骨质疏松组、正常组与骨折组之间vQCT及DXA各BMD差异均有统计学意义,在调节年龄、身高和体重因素后差异仍存在;骨质疏松组与骨折组之间只有小梁BMD及总体BMD差异有统计学意义,且小梁BMD下降幅度较大,达32.4%。结论 股骨近端vQCT测量比DXA能更敏感地反映绝经后女性股骨颈BMD的变化情况,为早期预测骨质疏松性骨折提供科学依据。

      Objective Multiple parameters of bone mineral density (BMD) and structure were determined by vQCT technique in the femoral neck of postmenopausal women, to evaluate the predictive ability of determined values. Methods Forty-seven postmenopausal women were collected and divided into three groups: nomal group 1 (26 cases), osteoporotic group 2 (14 cases) and vertebral fracture group 3 (7 cases). MSCT (GE Light Speed 16) scans of proximal femur were acquired and the reconstructed image slice thickness was 1.25 mm. The MPR images of proximal femur were performed using Osteo CAD software (Neusoft Medical System CO Ltd) to make measurements of femoral neck, including cortical bone, trabecular and integral BMD, neck length and min-area of the neck. The differences of all the indexes between the subjects were analysed by ANOVA. Results In all the volumetric BMD indexes there were statistically significant differences between group 1 and group 2, as well as group 1 and group 3, even after the correction of age, height and weight. In all the volumetric BMD indexes except the index of Cort, there were statistically significant differences between group 2 and group 3. The decrement of Trab between group one and group two was the most as 32.4%. Conclusion vQCT measurements of proximal femur can be confirmed as a better technique in assessing the changes of proximal femoral BMD than DXA, and can provide a scientific basis for the the early prediction of osteoporotic fracture.
