      目的 主要是通过肝纤维化和肝硬化阶段的CT灌注参数的变化,并与正常对照组及相应病理及血清学指标作对照,分析其血流动力学改变及病理基础以及与血清学指标的相关性。 方法 分别对39例肝纤维化患者、41例肝硬化患者及28例正常对照组行螺旋CT肝脏灌注扫描,用肝脏灌注分析软件取得不同病变阶段肝实质的灌注参数,包括肝血流量(BF)、肝血容量(BV)、肝动脉血流量(HAF)、平均通过时间(MTT)、表面通透性(PS),并与病理改变作对照研究和统计分析,同时对MTT、PS与血清学指标作相关分析。结果 正常对照组、肝纤维化组、肝硬化组BF及HAF为逐渐升高,MTT在肝硬化组缩短;正常对照组与肝纤维化组S3组间HAF升高;肝硬化Child-PughA、B、C分级中BV、BF、HAF逐渐升高,MTT在Child C明显缩短,PS在Child C升高;S2、S3、Child A、Child B、Child C组中HAF、 BV及BF值逐渐升高, MTT在Child C缩短而PS在Child C升高;对照组和Child-PughA、B、C分级中BV、BF、HAF逐渐升高,MTT缩短而PS升高;MTT 与透明质酸负相关,PS与IV型胶原正相关。 结论 随着纤维化程度的加重BV、BF、HAF逐渐升高,MTT缩短,PS升高。MTT、PS可反映胶原沉积及微循环改变情况。

      Objective To analyze the correlation among the change of hemodynamic, pathologic basis and serum index in liver fibrosis and cirrhosis from the point of CT perfusion index, and compare with control group, pathologic and serum index. Methods Dynamic spiral CT perfusion of the liver were obtained in 39 patients with liver fibrosis, 41 patients with cirrhosis and 28 control subjects. perfusion parameter of hepatic parenchyma in different disease stage including hepatic blood flow (BF), blood volume (BV), hepatic arterial fraction (HAF), mean transit time (MTT), permeability surface (PS) were obtained through liver perfusion analysis software, and contrast research with pathologic change and statistic analysis was done, at the same time, the correlation analysis among MTT, PS and serum index was made. Results BF、HAF、MTT were different among control group、liver fibrosis group and cirrhosis group, BF and HAF in three groups increased gradually, and MTT shortened in cirrhosis group. HAF were different between control group and liver fibrosis S3 group. BV, BF, HAF, MTT, PS were different among Child A, B, C of cirrhosis, BV, BF, HAF (Child A, Child B, Child C) increased gradually, MTT shorten in Child C, PS increase in Child C. BF, HAF, MTT, BV, PS were different among S2, S3, Child A, Child B, Child C, BV, BF increased gradually, MTT shortened in Child C. BV, BF, HAF, MTT, PS were different among control group and Child A, Child B, Child C, BF, BV, HAF increased gradually, MTT shortened. MTT was inverse correlation with HA, PS was direct correlation with IV collogen. Conclusion BF, HAF, MTT are different among control group, liver fibrosis group and cirrhosis group, BF, HAF increase gradually, MTT shorten. MTT, PS can reflect the deposition of collogen and the change of microcirculation.
