      目的 基于乳腺黏液癌的病理,探讨乳腺黏液癌的超声影像特征和肿瘤细胞分子生物学特性。方法 回顾性分析我院1999年1月-2006年5月经手术和病理证实的乳腺黏液癌56例的超声影像特征、病理表现及肿瘤分子生物学特性,用统计学方法比较黏液癌单纯型和混合型两组的超声影像特征和肿瘤分子生物学指标。结果 乳腺黏液癌是一种特殊类型的乳腺癌,近半数肿瘤呈膨胀性生长,富含黏液,肿瘤细胞异型性不明显,镜下分单纯型和混合型。以超声检查术前提示乳腺癌为正确诊断,诊断乳腺癌的敏感度为59%,单纯型和混合型的诊断敏感度分别为46%和71%(P=0.057)。本组11%的肿瘤超声误诊为纤维腺瘤。混合型的肿块不均匀、肿块后方声衰减和肿块内砂砾样钙化均高于单纯型(P<0.05)。混合型肿瘤分子生物学指标ER、PR和Ki67阳性率高于单纯型(P<0.05)。结论 乳腺黏液癌有特殊的组织学表现,导致部分肿瘤的声像图表现容易误诊为纤维腺瘤或因无明显的恶性征象而漏诊或误诊。混合型较单纯型具有更多的恶性肿瘤的超声图像特征,对激素治疗的敏感性也更高。

      Objective Based on the pathology, the sonographic characteristics and the molecular tumor markers of the mucinous carcinoma in breast were studied. Methods The data containing the history, sonography, pathology and the molecular markers of tumor cell with the mucinous carcinoma in breast of 56 cases in our hospital from Jan 1999 to May 2006 were retrospectively studied. The difference between the pure type and the compound type was studied by statistics. Results The mucinous carcinoma was one of the special kinds of carcinoma of breast in pathology. About half of tumors grew expansibily, and were full of mucus, and the heterocysts of the tumor cell were not typical. The two kinds both the pure and the compound were classified by microscope. The diagnostic sensitivities were 59%, 46% and 71% for the mocinous carcinoma, the pure and the compound respectively by sonography (P=0.057). Eleven percent of the cases was misdiagnosed as fibroadenoma by sonographic doctors. There were more hetarogeneous echoness, micro calcification in mass, acoustic attenuation behind mass in the compound than in the pure (P<0.05). The positive ratio of ER, PR and the Ki67 activities of the compound were higher than the ones of the pure (P<0.05). Conclusion The mucinous carcinoma is a special kind of carcinoma of breast in pathology, so some of them can be mistaken as fibroadenoma or be not thought as malignant tumor by ultrasonic doctors. The compound perhaps can be diagnosed as malignant tumor easier than the pure because the compound tumors have more typical malignant sonograhic characteristis and have more sensitivity to the hormone treatment.
