      目的 定量研究NBCA栓塞治疗食道胃静脉曲张后门静脉血流动力学的变化。方法 30例肝硬化反复上消化道出血患者,男性25例,女性5例。应用NBCA栓塞治疗食道胃静脉曲张,测定栓塞前后门静脉压力及门静脉灌注量的变化。结果 栓塞后30例患者门静脉压力由术前(33.90±8.09)cmH2O上升至术后(38.43±8.42)cmH2O(P<0.001)。脾静脉压力由术前(33.53±7.56)cmH2O上升至术后(36.87±7.87)cmH2O(P<0.001)。肠系膜上静脉压力由术前(33.13±7.16)cmH2O上升至术后(36.57±7.74)cmH2O(P<0.001)。术后1周12例患者门静脉灌注量由术前(0.7439±0.1627)ml·min-1·ml-1上升至术后(0.8830±0.1526)ml·min-1·ml-1(P<0.001),门静脉灌注指数由术前0.6129±0.0422上升至术后0.7202±0.0506(P<0.001)。结论 NBCA栓塞治疗食道胃静脉曲张后,门静脉压力升高,门静脉灌注得到改善。

      Objective To evaluate the hemodynamics change of portal system after transcatheter embolization with NBCA for the treatment of bleeding from esophagogastric varices. Methods Thirty patients (25 males,5 females) of liver cirrhosis with esophagogastric varices who suffered from repeated upper gastrointestinal bleeding were included in this study. Catheter insertion and embolization of esophagogastric varices were completed in all 30 patients. The common 5F-catheter was inserted into portal vein, splenic vein, superior mesenteric vein respectively and pressure were measured before and after transcatheter embolization. The portal vein perfusion and portal perfusion index of 12 patients were measured before operation and after one week. Results The pressure of portal veins significantly raised from (33.90±8.09) cmH2O before transcatheter embolization to (38.43±8.42) cmH2O after transcatheter embolization (P<0.001). The pressure of splenic veins significantly raised from (33.53±7.56) cmH2O to (36.87±7.87) cmH2O (P<0.001). The pressure of superior mesenteric veins significantly raised from (33.13±7.16) cmH2O to (36.57±7.74) cmH2O (P<0.001). The portal vein perfusion of 12 patients significantly raised from (0.7439±0.1627) ml·min-1·ml-1 before operation to (0.8830±0.1526) ml·min-1·ml-1 after one week (P<0.001). The portal perfusion index of 12 patients significantly raised from (0.6129±0.0422) to (0.7202±0.0506) (P<0.001). Conclusion The portal vein perfusion of the liver will be improved with the rising of the pressure of the portal vein after transcatheter embolization of esophagogastric varices.
