目的 采用16层螺旋CT对肾脏进行体积测量,探讨肝硬化时肾脏体积的变化规律及其与肝硬化严重程度之间的关系。 方法 对47例经临床资料、B超证实的肝硬化,其中肝功能 Child-Pugh分级A级16例,B级23例,C级8例以及36例正常组行16层螺旋CT容积扫描,采用仪器自身携带的软件,分别测量左侧肾脏皮质、髓质及实质体积,计算皮质/实质的百分比并研究其变化规律。 结果 正常组左侧肾脏的实质体积为(160.01±39.02)cm3,皮质体积为(90.01±18.21)cm3,髓质体积为(70.00±15.74)cm3,皮质/实质体积比值为56.3%±9.10%,肝硬化时,其中皮质体积与正常组间有统计学差异,且Child-Pugh A、B、C级左侧肾脏体积的变化表现为皮质逐级缩小,髓质变化不明显。 结论 16层螺旋CT能准确地测量肾脏实质和皮质、髓质的体积,肝硬化肾脏皮质的大小与疾病的严重程度有一定相关性。 |

Objective To measure the volume of kidney with 16 slices helical CT, and to discuss the relationship between the changes of the renal volume and the severity of liver cirrhosis. Methods A volume CT scan was performed in 81 participants, including 47 patients with liver cirrhosis demonstrated by clinica1, ultrasound for liver cirrhosis group and 36 patients who were normal in the liver as control group. The liver cirrhosis group was divided into three subgroups depending on their liver functions, including 16 cases in Child-Pugh A, 23 cases in Child-Pugh B, and 8 cases in Child-Push C. The volume of the left renal cortex, renal medulla and parenchyma were measured respectively, and the percentage of cortical volume/parenchymal volume in the kidney was also calculated. Results The parenchymal volume of the left kidney in the control group was (160.01±39.02)cm3, cortical volume (90.01±18.21)cm3, and medullary volume (70.00±15.74)cm3. The percentage of cortical volume/parenchymal volume in the left kidney was 56.3%±9.10%. There was statistical significance between cirrhotic liver and the control group of the left cortical volume. The changes of the renal volume in Child -Push A, B, C cirrhotic liver were that the cortical volume became shrinked gradually and the medullary volume had no apparent changes. Conclusion The 16 slices helical CT can measure the renal volume accurately, and the changes of the cortical volume are correlated with the severity of liver cirrhosis. |
