目的 探讨三维容积超快速多期动态(LAVA)增强扫描序列在乳腺磁共振检查中的应用价值。方法 38例临床疑似或钼靶提示为乳腺肿瘤的病人行MRI平扫及动态增强扫描,其中18例采用LAVA动态增强扫描序列,其余20例采用FAME动态增强扫描序列,对比分析两种动态增强扫描序列的图像质量及其对病灶的显示,并采用4分法评分。结果 4分法原则评分,LAVA组:4分13例(72.2%),3分4例(22.2%),2分1例(5.6%),无1分病例;FAME组:4分3例(15%),3分11例(55%)2分5例(25%),1分1例(5%)。肿瘤血管显示情况,LAVA组:肿瘤供血动脉的来源动脉分支、异常增粗段及引流静脉清晰显示且走行连续,管径小于2 mm的血管得以显示;FAME组:肿瘤供血动脉异常增粗段及引流静脉显示,管径小于2 mm的血管显示困难,且走行连续性较差。结论 LAVA动态增强序列图像质量优于FAME序列,能更清楚地显示乳腺肿瘤的形态、边缘、内部强化特点及其供血血管。 |

Objective To discuss the application value of liver acquisition with volume acceleration (LAVA) dynamic enhanced magnetic resonance image in the examination of breast tumours. Methods Thirty-eight cases with clinical or mammographical suspicion of breast lesions were performed MRI examination. Eighteen of them were performed Gd-DTPA LAVA dynamic contrast-enhanced MRI, and the rest 20 cases were performed Gd-DTPA FAME dynamic contrast-enhanced MRI.The blood vessel of breast toumur and surrounding Lymph nodes were observed, and the image quality of the two contrast-enhanced methods was compared and scored according to the same 4 scores evaluation standard. Results According to the 4 scores evaluation standard, in group LAVA: 13 cases (72.2%) were rated 4; 4 cases (22.2%) were rated 3; 1 case (5.6%) was rated 2; no case was rated 1. In group FAME: 3 cases (15%) were rated 4; 11 cases (55%) were rated 3; 5 cases (25%) were rated 2; 1 case (5%) was rated 1. For the display of tumour vascularity,in group LAVA: The branches of the toumors’ supply arteries, the widened segment of them,the drain veins and the paths of the vessels were displayed clearly; The blood vessels less than 2 mm were shown as well. In group FAME: The widened segment of supply arteries and the drain veins were demonstrated,but FAME showed the blood vessels less 2mm and their pathes poorly. Conclusion The image quality of the LAVA dynamic contrast-enhanced MRI was better than FAME dynamic contrast-enhanced MRI. The modality, edge, enhanced characteristic of inside and the blood-provided vessel of breast toumor can be delineated satisfactorily by Gd-DTPA LAVA dynamic contrast-enhanced MRI. |
