目的 评价ZQL型可回收式腔静脉滤器预防肺动脉血栓栓塞的安全和有效性。 方法 144例具有下腔静脉滤器植入适应证患者,常规经股静脉(37)或颈静脉(107)置入144枚滤器,滤器置入后定期腹部平片和下腔静脉彩超评价下腔静脉和滤器位置、通畅情况。滤器植入后如果下肢深静脉血栓在2周内清除,则取出滤器。取出前分别摄腹部平片及行下腔静脉、肺动脉造影。否则该滤器将作为永久滤器长期留置后并继续进行随访观察。 结果 144 滤器均成功置入预定部位,未发生滤器展开不良。置入技术成功率为100%,一次置入成功率为98.61%,2 次植入成功率为100%。随访成功137例(随访率为95.14%),其中43枚滤器于置入后7~14天(中位12天)取出。余94例随访7天~39个月(中位随访期为17个月),未发生滤器移位、倾斜、滤器断裂或滤器穿透静脉壁造成血肿并发症。无下腔静脉闭塞发生,随访期内全部患者未再发生症状性肺动脉栓塞。 结论 本研究结果表明ZQL型可回收式腔静脉滤器预防肺动脉血栓栓塞是安全、有效的。其独特的支架式设计具有稳定性好、捕获效能高、置入与取出容易,符合临床需求的特性。 |

Objective To assess the safety and effectiveness of ZQL-retrievable vena cava filter (ZQL-RVCF) for the prevention of pulmonary embolism (PE). Methods One hundred and forty-four ZQL-RVCFs were implanted into 144 patients with indication of vena cava filter implanting via femoral vein (37) or right internal jugular vein approach (107). Abdominal radiograph for the location of ZQL-RVCFs and duplex scanning of inferior vena cava (IVC) were made as schedule. For the patients with complete clot removal of lower extremity deep vein thrombosis within 2 weeks, ZQL-RVCF was retrieved and abdominal radiograph,inferior vena cavography and plumonary angiography were carried out at the time of filter retrieving,otherwise the ZQL-RVCF was permanently placed in the IVC and under surveillance. Results One hundred and forty-four ZQL-RVCFs were successfully placed in proper position. Technical success rate was 100%. The first time and the second time implanting technical success rate reached 98.54% and 100%,respectively. One hundred and thirty-seven patients were followed up (follow up rate 95.14%). Forty-three ZQL-RVCFs were retrieved at 7—14 days (the median time was 12 days) respectively after placement. The remaining 94 patients were followed up for 7 days to 39 months (the median time was 17 months) and there were no filter migration,filter tilting,filter fracture,IVC perforation,IVC thrombotic obstruction and symptomatic PE. Conclusion This study demonstrates the ZQL-RVCF is a safe and an effective device. Its exclusive stent-shape structural characteristics ensure its good stability,anti-tilting,efficacy of capture,and convenience for insertion and retrieval. It is an option for satisfying the clinical demand for preventing PE. |
