      目的 评价不同流速对比剂注射方式对64层CT冠状静脉成像质量的影响。方法 将25例志愿者分成A(n=13)、B(n=12)两组。分别给予对比剂速率3.5 ml/s和4.5 ml/s进行64层MSCT冠状静脉CTA检查。分析比较两组的冠状静脉及相伴性冠状动脉的显示率,显示积分及CT值。结果 4.5 ml/s 组CT值优于3.5 ml/s ,两组心大静脉,心中静脉,左室后静脉及右冠状动脉远段的CT值有显著统计学差异(P<0.05)。两组冠状静脉的显示率均为100%。两组的冠状动脉,冠状静脉显示评分有统计学差异(P<0.05)。结论 采用流速4.5 ml/s显示冠状静脉及冠状动脉总体优于3.5 ml/s,更适合显示动静脉关系。3.5 ml/s流速更适合心功能不全的患者。

      Objective To assess the effects of the injection rate on the enhancement of coronary veins using 64-slice CT. Method Twenty-five patients without heart disease were divided randomly into group A (n=13) and group B (n=12). The injection rate of the contrast media was 3.5 ml/s and 4.5 ml/s respectively. A single-slice dynamic scan was performed at the level of coronary sinus to define the circulation time from coronary arteries to coronary sinus. Second, bolus tracking was used to carry out MSCTCV and an ROI was plotted inside the ascending aorta. The triggering threshold was 100 Hu. TD (transition delay) was equal to the coronary circulation time. All patients underwent 64-MSCT (Sensation 64; Siemens, Germany) The mean injection dose was (70.69±7.54) ml or (83.5±7.40) ml according to the scan time with 40 ml saline flush. The CT value, display rate and degree of the coronary veins (CS, GCV, MCV, PVLV) and of the relative arteries (LCX, distal RCA, PDA) were compared between the two groups. The display degree of vessels was evaluated using a 0-3 point scale. Results The CT value in the CV and CA of 4.5 ml/s was superior to 3.5 ml/s. The mean CT value of the coronary vessels was (165.02±56.07)-(283.71±56.63) Hu and (137.31±31.07)-(236.55±70.34) Hu respectively. There was a significant difference in the CT values of GCV, MCV, LPV and distal-RCA (P<0.05). The display rate of the coronary veins was 100% for two groups. The average score of CV and CA was 2.75, 2.18 in group A and 2.96, 2.53 in group B. There was statistical significance in the display score of CV and CA (P=0.041, 0.029). Conclusion 4.5 ml/s is superior to 3.5 ml/s injection rate for 64-slice CT coronary venography; injection rate is more suitable for patients with advanced heart failure.
