      目的 探讨实时三维超声心动图(RT-3DE)结合半自动边界探测技术测量正常和异常左室容积及收缩功能的可行性、准确性及可重复性。 方法 健康自愿者20例,冠心病患者25例纳入统计,应用Philips iE33成像系统采集三维数据,使用Tomtec 4D LV-Analysis CAP2.5软件脱机测量左室容积参数,包括舒张末期容积(EDV)、收缩末期容积(ESV)、每搏量(SV)和射血分数(EF),并与二维超声心动图(2DE)的双平面Simpson’s法测值比较。 结果 ①正常组和冠心病组中,RT-3DE与Simpson’s法所测EDV、ESV、SV和EF差异均无统计学意义且一致性好(P>0.05)。②两组中,两法所测EDV、ESV、SV和EF高度相关,但冠心病组相关系数均较正常组相应测值有所降低。③两法的观察者间变异在正常组中均小,冠心病组较正常组有所增加,且冠心病组中Simpson’s法的观察者间变异增加更为明显,大于RT-3DE法的观察者间变异。结论 RT-3DE结合半自动边界探测技术测量左室容积及收缩功能的方法简便、可行、准确、重复性高,在临床上具有推广应用的价值。
英文摘要:
      Objective To explore the feasibility, accuracy and reproducibility of measuring normal and abnormal left ventricle volumes (LV) and systolic function using real-time three-dimensional echocardiography (RT-3DE) combined with semi-automatic border detection method. Methods Twenty normal volunteers and 25 patients with coronary artery disease (CAD) were studied, and all the three-dimensional echocardiographic datasets were acquired by Philips iE33 medical system. Then a series LV parameters, namely, end diastolic volume (EDV), end systolic volume (ESV), stroke volume (SV) and ejection fraction (EF), were offline measured by Tomtec 4D LV-Analysis CAP2.5 software. All values from RT-3DE were compared with corresponding values calculated by biplane Simpson’s method using two-dimensional echocardiography (2DE). Results ①In both normal group and CAD group, no statistical differences were found between RT-3DE and Simpson’s method in EDV, ESV, SV and EF calculations (P>0.05). And values from the 2 Methods were consistent with each other. ②In both groups, EDV, ESV, SV and EF measured by RT-3DE correlated strongly with those measured by Simpson’s method. But the correlation coefficients of CAD group were lower than those of normal group. ③In normal group, inter-observer variability of EDV, ESV, SV and EF measured by both Methods were small. While in CAD group, inter-observer variability of the 2 Methods increased, and inter-observer variability of Simpson’s method increased markedly, which was much larger than that of RT-3DE. Conclusion The new assessment method of RT-3DE combined with semi-automatic border detection method can conveniently, feasibly, accurately and high-repeatedly measure LV volume and systolic function, and was suitable for clinical applications.
