| 目的 探讨炎症型支气管肺泡癌的CT表现的动态发展演变规律,提高CT诊断的临床价值。方法 回顾分析26例病理学证实的并属于影像学分型炎症型的支气管肺泡癌患者不同时间点的系列CT随访图像,总结不同时间的CT表现特征及其动态演变规律,探索CT诊断的典型影像学征象。结果 炎症型支气管肺泡癌相对典型的CT征象有:①毛玻璃征象背景上合并腺泡结节样实变;②无典型分布特征的大小不等的边缘模糊的叶段性实变;③实变基础上的病理性空气支气管充气征;④动态演变特点包括范围逐渐扩大、形态逐渐多样、密度逐渐密实。结论 CT特别是两次以上不同时期的随访CT检查是炎症型支气管肺泡癌的理想诊断手段。 |

| Objective To investigate the CT features and its dynamic developmental regularities of the pneumonic type of the bronchioalveolar carcinoma (BAC). Methods According to the radiological classification, the serial CT images taken in different follow-up intervals of 26 cases of the pneumonic type of BAC pathologically confirmed have been reviewed retrospectively. In order to probe into the pathognomonic CT features of the pneumonic type of BAC, the characteristics of its CT findings in different times and its dynamic development have been analyzed. Results The relative CT pathognomonic characteristics of this type of BAC were as follows: ①the acinar nodules on a background of ground glass opacity; ②the random distributed varied sized lobe and/or segment consolidation with blurred marges; ③the consolidation accompanied with the air bronchogram sign; ④the dynamic developmental characteristics were including: the lesion range spreading gradually, the lesion shape growing multiplicities, and the density of the lesion gradually turning into more high. Conclusion The CT, especially more than two times of CT taken in different follow-up time, is an ideal diagnostic method for the pneumonic type of BAC. |
