      目的 探讨能量多普勒超声检测乳腺肿瘤血管的准确性及误差原因分析。方法 对73例灰阶超声诊断为乳腺肿瘤的患者行能量多普勒检查,采用图像分析软件测定肿瘤区域内的彩色像素密度,与病理血管密度对照。采用多元回归方法分析患者年龄、肿瘤的大小、距乳头距离以及后方回声衰减等对相关性研究的影响。结果 ①乳腺肿瘤彩色像素密度与病理小血管密度的相关系数为0.55(P<0.01)。②后方回声衰减是影响相关性的最主要因素(P=0.007),年龄次之(P=0.011), 肿瘤大小和距乳头距离无显著影响(P值均大于0.05)。③校正后的彩色像素密度与病理小血管密度相关系数为0.75(P<0.01)。结论 能量多普勒检测乳腺肿瘤血管的准确性主要受肿瘤后方回声衰减的影响,如以浅表部的彩色像素密度代表整个肿瘤的彩色像素密度,可以提高其准确性。

      Objective To analyze the accuracy and the source of error of power Doppler ultrasonography in assessing breast tumor vascularities. Methods Power Doppler ultrasonography was performed in seventy-three patients with breast tumors which were diagnosed by gray scale ultrasonography. The Doppler signal intensity in the tumor region was evaluated by professional imaging analytical software using the color pixel density and correlated with pathologic vessel density. Multiple regression was used to compare the influence of age, tumor size, distance to the nipple and acoustic attenuation on correlation. Results There was positive correlation between the small vessel density and the color pixel density of breast tumors (r=0.55, P<0.01). Regression models showed acoustic attenuation was the significant factor influence on the correlation (P=0.007) and age has a weak influence on the correlation (P=0.011). There were no significant influence of tumor size and the distance to the nipple on the correlation (P>0.05). After emendation, the correlation between the color pixel density and small vessel density was improved (r=0.75, P<0.01). Conclusion The accuracy of power Doppler ultrasonography in assessing vascularities of breast tumor was mainly affected by acoustic attenuation. The accuracy was improved after the color pixel density of the whole breast tumor was replaced by the superficial color pixel density for breast tumor with acoustic attenuation.
