目的 建立大鼠腰椎间盘针刺退变模型并利用影像学方法进行评价。方法 32只SD大鼠,对其中20只进行腰椎间盘纤维环厚度测量,以确定针刺深度;另12只使用21 G 微量穿刺针对L3/L4、L4/L5及L5/L6椎间盘纤维环行部分或全层针刺,术前及术后4周、8周行X线及MRI检查,然后行椎间盘病理组织学检查。 结果 测量大鼠腰椎间盘纤维环厚度后,确定适用于L3/L4、L4/L5及L5/L6椎间盘纤维环部分及全层针刺的深度分别为1.5 mm和2.3 mm。X线检查纤维环部分及全层针刺组可见不同程度的腰椎间隙变窄,椎体前缘唇缘样增生,脊柱生理弯曲异常等改变,其中以纤维环全层针刺组为明显;而正常对照组未见椎间隙变窄、椎体前缘骨质增生等征象。MRI检查纤维环全层针刺4周、部分针刺8周后椎间盘信号在 T2WI 上信号强度不同程度的降低、椎间盘膨出甚至突出、硬膜囊受压等征象,正常对照组的椎间盘未见信号改变及膨出或突出征象。病理组织学检查发现纤维环部分针刺组及全层针刺组椎间盘于术后4周时即发生退变,而全层针刺组退变较严重。结论 利用针刺纤维环的方法可以成功建立大鼠椎间盘退变模型,其可控性强、重复性好、创伤小;影像学方法可对椎间盘退变进行早期精确、动态评估,评价椎间盘退变动物模型建立的成功与否。 |

Objective To establish a reproducible rat model of disc degeneration by puncturing the annulus with needles of defined gauges, and to evaluate the effect by medical imaging. Methods Total 32 skeletally mature male Sprague-Dawley rats, 20 were used to determine the depth of puncture by measuring anular thickness of lumbar disc, 12 rats were carried out transperitoneal ventral operation, and anular stab was performed using 21-gauge needle to make a full-thickness or partial-thickness stab incision in the L3/L4、L4/L5 and L5/L6 anteriorlateral disc. X-ray and MRI were performed before the operation and at 4, 8-week after operation, and histological analysis were performed. Results The depth of puncture was respectively partial-thickness 1.5 mm and full-thickness 2.3 mm. There was no special radiological characterization after operation in control group, however,in X-ray films, lumbar intervertebralspaces stenosis and vertebral bodies unstability exist in the experimental group. The stabbed discs exhibited a decrease signal intensity from T2-weighted images starting at 4-week time point in full-thickness stab incision and 8-week in partial-thickness. Histological analysis revealed disc degeneration in both stab incision groups at 4-week time point and full-thickness stab incision displayed severely changes compared with partial-thickness. Conclusion The method is reliable, effective and reproducible to establish a rat model of intervertebral disc degeneration by anulus stab incision with needles of defined gauges. It offers model to study the mechanism of lumbar intervertebral disc degeneration. Medical imaging, including X-ray and MRI, can evaluate the early disc degeneration,and to evaluate the effect of degeneration model. |
