      目的 研究1H-MRS在肌肉脂肪含量的定量分析中的价值及与病理分级的相关性。方法 使用GE 1.5 T MR扫描机及正交birdcage动物线圈(50 mm),对28只成年Wistar大鼠左侧后肢比目鱼肌进行1H-MRS采集。采集到的波谱使用SAGE软件进行后处理,分析细胞内脂肪和细胞外脂肪含量。使用电镜观察肌肉细胞内的脂滴含量。结果 全部大鼠均成功采集肌肉波谱。所有谱线均可以解析位于3.02~3.05 ppm处的总胆碱,1.3 ppm处的肌肉细胞内脂肪和1.5 ppm处的肌肉细胞外脂肪。大鼠肌肉细胞内脂肪含量/总胆碱比值为0.58~5.72,与电镜下观察到的细胞内脂肪的含量呈正比(r=0.952,P<0.001)。结论 使用1H-MRS可以准确、无创测量肌肉细胞内的脂肪含量,且与病理检查结果一致。

      Objective To evaluate the value of 1H-MRS in the muscular fat quantification and the relationship between the muscular fat content determined by 1H-MRS with the degree of fat content determined by electron microscope. Methods The spectrums in the soleus muscle of the left hind leg were collected in 28 adult Wistar rats. Total Cretine (tCr) was chosen as internal reference and area under the peak of tCr (StCr) was integrated, areas under the peak of intramyocellular lipid (IMCL) and extramyocellular lipid (EMCL) were integrated as SIMCL and SEMCL, Ratio of SIMCL/StCr and SEMCL/StCr were regarded as IMCL and EMCL, respectively. Electron microscope was used to determine IMCL and EMCL pathologically. Results Spectrums in all of the Wistar rats were successfully collected. Muscular 1H-MRS spectrum can resolve the peak of tCr at 3.02-3.05 ppm, IMCL at 1.3 ppm and EMCL at 1.5 ppm. The ratios of SIMCL/StCr ranged from 0.58 to 5.72, and were strongly correlated with IMCL revealed by electro microscope (r=0.952, P<0.001). Conclusion 1H-MRS can be used to accurately quantify the intramyocellular lipid.
