目的 用多层螺旋CT灌注成像技术观察肝硬化患者肾脏血流灌注参数的变化。方法 对20例正常对照组和47例肝硬化病例组,其中代偿期肝硬化(compensated cirrhosis, CC)18例,失代偿期肝硬化(decompensated cirrhosis, DCC)29例,行16层螺旋CT肾脏灌注检查。通过专用灌注软件得出兴趣区相应的灌注参数值,包括血流量(blood flow, BF),血容量(blow volume, BV),对比剂平均通过时间(mean transit time, MTT)和表面渗透积乘积(permeability surface area product, PS)。统计分析这些参数。结果 DCC较CC和正常对照组BF值明显减低,MTT值明显延长 ;DCC较CC和正常对照组皮质的PS值明显减低 ;CC与正常对照组BF值、MTT值、PS值差别无统计学意义(P>0.05)。3组间的BV值和髓质PS值差别无统计学意义(P>0.05)。结论 失代偿期肝硬化患者肾脏BF值和皮质PS值明显减低,MTT值明显延长,而BV值和髓质PS值无明显变化。 |

Objective To study the changes of renal perfusion parameters with liver cirrhosis by 16 slices spiral-CT. Methods Dynamic contrast-enhanced single-location sequences CT scans of the kidney were obtained in 67 individuals who included 20 control subjects,18 patients with compensated cirrhosis (CC) and 29 patients with decompensated cirrhosis (DCC) proven by clinical. These data were processed on GE AW 4.1 workstation using kidney perfusion agreement of perfusion 3 software package. Renal perfusion parameters such as renal blood flow (BF), blood volume (BV), mean transit time (MTT), permeability surface area product (PS) were calculated. Statistics treatment made SPSS 11.5 software. Results Compared with control group and CC group, the BF and MTT of DCC group were significantly altered . Compared with control group and CC group, PS of the renal cortex were significantly decreased in DCC group . BV and medulla PS of three group had not significant difference . Conclusion In DCC BF of kidney and PS of cortex renal were decreased obviously, and MTT was significant increased, but BV of kidney the PS of medulla renal had notobviously change. |
