目的 探讨彩色多普勒血流显像在观察肱骨骨折愈合过程的价值。方法 用彩色多普勒超声对65例肱骨骨折患者在内固定术后第1、2、3、4、9及第15周内依次观察患者骨折骨痂形成的声像特征及其血运情况,并与X线摄片所见进行对比分析。结果 在血肿机化期的第2~4周,肱骨骨痂形成良好的患者,彩色多普勒超声可清晰地观察到骨折周围的纤维骨痂,其血流阻力指数RI值随时间而下降(P<0.05),CDFI分布在 II~III;肱骨骨折愈合不良患者,骨折周围的纤维骨痂显示不佳,血流阻力指数RI值随时间变化不大(P>0.05),CDFI分布在0~I。肱骨骨痂愈合良好组与愈合不良组的血流阻力指数RI值相比较有显著统计学差异(P<0.05)。结论 应用彩色多普勒血流显像观察肱骨骨折愈合过程,简便易行,无痛苦,是一种行之有效的早期预测肱骨骨折愈合前景的方法。 |

Objective To investigate the value of colour Doppler flow imaging (CDFI) on the observation of humeral shaft fractures. Methods Sixty-five patients with humeral shaft fractures treated with internal fixation were examined sonographically with color Doppler ultrasonography (CDU) during the 1st week, 2nd week, 3rd week, 4th week, 9th week and 15th week after the surgery when radiographic demonstration may confirmed the callus' consolidation. Then the resistance indexes (RI) of the blood flow among the good healing and bad healing groups were compared respectively during the different durations of the observation. Results Within the first month after the operation, especially during the 2nd week to the 4th week, for those who had a good humeral shaft fracture healing, the fibrous callus around the fracture sites with CDFI signals graded Ⅱ-Ⅲ and gradually decreased RI (P<0.05) were founded by CDFI; while for those who underwent delayed fracture healing, lack of fibrous callus around the fracture sites with CDFI signals graded 0-Ⅰ and persistent high RI (P>0.05) were founded by CDFI. Significant differences were found about RI in the above two groups during different weeks after the operation (P<0.05). Conclusion CDFI is an early easy-to-perform painless and effective method to predict the humeral shaft fracture healing. |
