| 目的 分析附件区病变的超声造影时间-强度曲线在形态和参数上有无差异,初步评估其对于鉴别附件区病变良恶性的诊断价值。方法 选择超声造影后病灶内部有增强成分的36例附件区包块(良性16例,恶性20例)进行分析。选择增强区作为感兴趣区获得TIC,将曲线下降支分为单相和双相。比较良恶性肿物在曲线下降支形态和曲线各参数中有无差异。结果 ①曲线下降支形态分析(共29例):分为单相和多相,单相组中恶性病变占90.9％,良性病变占9.1％;双相组中良性病变占61.1％,恶性病变占38.9％。两组差异有显著性(P＝0.008)。以此指标作为诊断病变良恶性的诊断准确率为72.4%;与常规超声联合应用,诊断准确率为75.9％。②曲线参数分析:当恶性组仅包括卵巢原发癌和转移癌时,在半洗出时间和半廓清时间上显著大于良性组。结论 超声造影时间-强度曲线对于附件区肿物的良恶性鉴别诊断有一定的辅助作用,具有一定的临床应用价值。 |

| Objective To compare the shape and kinetic parameters of time-intensity curve (TIC) of contrast-enhanced ultrasound (CEUS) between benign and malignant adnexal lesions, and to evaluate its efficiency in differentiation the two types of lesions. Methods Thirty-six patients with adnexal lesions, including 16 benign and 20 malignant ones, were performed with CEUS. The regions of interest (ROIs) were chosen on the enhancement parts in the lesions in order to obtain the TIC curves. The shape of the wash-out curve (descending branch) of TIC was divided into two patterns: mono-phase or bi-phase pattern. The kinetic parameters of TIC were compared between benign and malignant lesions. Results ①90.9% of mono-phase lesions were malignant; 61.1% of bi-phase lesions were benign (P=0.008). The diagnostic efficiency made by TIC wash-out curve was 72.4%, which was improved to 75.9% combined with baseline US. ②When the malignant group including only ovarian carcinomas, the half-washout time and half-expurgation time were significantly longer than benign group. Conclusion TIC of CEUS may help for differentiating benign and malignant adnexal lesions. |
