目的 探讨99Tcm-HL91(4,9-二氮-2,3,10,10-四甲基十二烷-2,11-二酮肟)显像与肺癌恶性程度、肿瘤微血管密度(MVD)表达的关系。方法 35例非小细胞肺癌患者,注射99Tcm-HL91,4 h后进行胸部前后位、后前位平面及断层显像,利用感兴趣区技术,计算肿瘤与对侧(T/N)正常组织的放射性比值。所有患者均行细胞学检查,将标本做HE染色、免疫组织化学染色。结果 99Tcm-HL91平面显像:35例非小细胞肺癌患者中的30例显像阳性;断层显像:33例显像阳性。定量分析:断层像4 h的T/N比值显著高于平面像的T/N比值(t=10.619,P=0.000);HL91显像的T/N比值与肺癌细胞恶性程度相关(r=0.626,P=0.000)。癌巢及癌细胞密集区的MVD显著少于癌周区及增生活跃区。MVD表达在非小细胞肺癌细胞恶性程度分级间有显著差异(F=8.49,P=0.01)。HL91显像的T/N比值与MVD表达高度相关(r=0.721~0.862,平均r=0.791,P<0.05)。结论 99Tcm-HL91显像的T/N比值与非小细胞肺癌细胞恶性程度分级呈中度相关,与MVD表达呈高度相关。99Tcm-HL91显像可以作为一种无创伤的检查方法,评价肿瘤分级,并对NSCLC生物学行为的了解和预后判定有着重要的意义。 |

Objective To evaluate the ability of 99Tcm-HL91 imaging in characterising the non-small-cell lung cancer (NSCLC) malignant grades and the relation between 99Tcm-HL91 (4,9-diaza-3,3,10,10-tetramethyldodecan-2,11-dionedioxime) imaging and MVD expression by comparing imaging with HE、immunohistochemistry staining in human NSCLC. Methods Thirty-five patients,with NSCLC, performed SPECT and anteroposterior、posteroanterior chest planar imagings at 4 hour after injection of 99Tcm-HL91. ROIs were drawn in imagings of tumor and symmetry in lung planar scan, and the radioactivity ratios tumor-to-normal tissue were calculated. All patients were operated and the samples were stained for immunohistochemistry. Results Planar imaging: 99Tcm-HL91 was showed visible uptake in 30 out of 35 NSCLC patients. SPECT imaging: 33 out of 35 were positive. Tumor-to-normal tissue ratios (T/N), the difference of T/N between between planar imaging and SPECT was significant (t=10.619,P=0.000). And uptake of 99Tcm-HL91 was related to the malignancy of the cancer cell (r=0.626,P=0.000). MVD was higher expressed at the periphery of tumor than in the middle of tumor. The difference of expressive intensity among the malignancy of cancer cell was significant (F=8.49,P=0.01). Uptake of 99Tcm-HL91 was strongly correlated to the expressive intensity of MVD (r=0.721-0.862,mean r=0.791,P<0.05). Conclusion Uptake of HL91 and expression of MVD were strongly correlated to the malignancy of the cancer cell. HL91 imaging is an important atraumatic method in predicting the malignancy of the cancer cell and the therapeutic effect. |
