| 目的 分析腰肌病变累及肾旁后间隙的CT表现,探讨其侵犯途径和解剖学基础。方法 回顾性分析21例CT扫描资料和临床资料完整的腰肌病变累及肾旁后间隙的连续性病例,其中经手术病理证实15例;经临床综合指标及抗炎治疗随访显效而诊断者6例。着重观察:①腰肌病变累及肾旁后间隙的CT表现;②腰肌病变累及肾旁后间隙的途径。结果 ①肺癌腰肌转移2例,肝细胞癌腰肌转移1例,胃癌腰肌转移1例,腰肌肉瘤1例,腰肌黏液腺癌1例,淋巴瘤侵犯腰肌1例,均为单侧;腰椎结核并腰肌冷脓肿10例,其中5例为双侧;腰肌脓肿4例,均为单侧;②腰肌病变累及肾旁后间隙的途径为:直接侵犯同侧肾旁后间隙或先侵犯同侧肾周间隙后突破肾后筋膜侵犯肾旁后间隙或两种方式混合出现;③腰肌病变累及肾旁后间隙表现为:与腰肌病变相连的肿块影;肾旁后间隙内结节影、索条影;肾旁后间隙内密度增高;肾后筋膜增厚。结论 CT可准确显示腰肌病变累及肾旁后间隙的解剖学细节。 |

| Objective To analyze the CT features of psoas diseases involving the posterior pararenal space and to determine the spreading pathways and anatomic bases. Methods Twenty one cases with psoas diseases were reviewed retrospectively,of which 15 cases were proved by surgery and pathology, 6 cases were proved by clinical and laboratory findings. Emphasis was placed on the CT imaging features of the involvement of the posterior pararenal space and the spreading pathways. Results ①Four cases were metastatic tumors from lung carcinoma (n=2) and hepatocellular carcinoma and gastric carcinoma, sarcoma one case, mucus adencarcinoma one case, lymphoma one case. Ten cases were tuberculous cold abscesses of which 5 cases were bilateral; 4 cases were abscesses. ②The spreading ways that the psoas diseases involved the posterior pararenal space were direct and/or indirect through the perirenal space. ③The CT features of the involvement of the posterior pararenal space included mass, nodus, stripe shadow, enhancement of the density, thicking of the posterior renal fascia. Conclusion CT is the imaging method of choice to depict clearly the posterior pararenal space involvement by the psoas diseases. |
