目的 研究正常子宫1H波谱谱线特点及与月经周期的相关性。方法 10位月经周期正常的健康女性志愿者,分别在月经三个不同时期进行磁共振检查:月经周期的第4~7天(经后期);第13~16天(排卵期); 第26~30天(经前期)。采用1.5T磁共振扫描仪,同时进行常规T1、T2像扫描和MRS检查。MRS采用PRESS技术,单体素采集,短TE (144 ms)。为最大限度减少操作因素影响,限制MRS体素位置及大小,匀场后带宽和抑水程度。以信噪比(SNR)作为衡量谱峰高低的标准。结果 胆碱峰(Cho)在所有检查对象中均出现,且峰高随月经周期的不同时相而变化,经前期信噪比最高,经后期最低。脂峰(Lip)在部分谱线中可见,在经后期出现较多且峰值较高。乳酸和肌酸峰偶尔出现,SNR较低。结论 正常子宫磁共振波谱中两个明显的化合物峰是胆碱和脂质,且峰高随月经周期的不同时相变化,推测可能主要来源于有周期变化的内膜层。此表现提示在研究子宫病变波谱时应谨慎,子宫内膜周期可能对谱线产生影响。 |

Objective To investigate the 1H MR spectroscopic features of normal uterine and its relationship with menstrual cycle. Methods Ten generative healthy female volunteers underwent pelvic MR scanning at three different time of their menstrual cycle: postmenstrual phase (immediately after menstruation, 4-7th day of the cycle), ovulatory phase (13-16th day) and premenstrual phase (just before menstruation, 26-30th day) by a 1.5T MR scanner. Routine MR images and MR spectroscopy were acquired. MRS was performed with 8-channel phased-array body coil, single voxel, by the point resolved spectroscopy (PRESS) technique with a short TE (144 ms). To exclude the influence of artificially related factors as much as possible, voxel placement and volume, full width at half maximum and water suppression rate after shimming, were restricted. Set signal to noise ratio (SNR) as the semiquantitative measurement index of certain peak height in MRS line. Results Choline peak was the one that could be detected in every case, while its height varies according to different time points of a menstrual cycle, highest in premenstrual phase, lowest in postmenstrual phase. Lipid signal could be detected in some cases, with higher ones appears often in postmenstrual phase. The lactate and creatine signals were not so confidentially detected. Conclusion The two most common compound peaks in normal uterine MRS are choline and lipid. While assumed from their special cycle related change, they may mainly derived from endometrium. Such results give us a clue on prudently taking the two compounds as indicators of uterine tumors, for they may affected by menstrual cycle change. |
