      目的 探讨MR弥散加权成像(MR-DWI)检测早期肝纤维化及定量肝纤维化程度的能力。方法 对以CCl4诱导产生的肝纤维化模型兔和对照兔行MR常规扫描及DWI检查,以病理学肝纤维化分期为基础将其划分为无肝纤维化组(S0)、轻中度纤维化组(S1~S2)和重度纤维化/肝硬化组(S3~S4),在b取300、600、1000 s/mm2时,比较各组DWI图像信号强度(SI)、表观弥散系数(ADC)和指数表观弥散系数(EADC)的变化情况。结果 在b取300、600 s/mm2时,无肝纤维化组、轻中度纤维化组和重度纤维化/肝硬化组的SI与EADC值依次升高,ADC值依次降低,差异存在统计学意义(P<0.01);在b取1000 s/mm2时,仅SI值随肝纤维化组别升高而升高(P<0.01)。b取300 s/mm2时,ADC与EADC值三组间两两比较差异均有统计学意义(P<0.01)。结论 初步实验研究表明,肝纤维化时肝内水分子弥散受限,且随肝纤维化程度增高,水分子弥散能力逐渐减低。在选择合适b值的情况下,MR-DWI可成为检测早期肝纤维化及定量肝纤维化程度的有效手段。

      Objective To investigate the ability of diffusion-weighted MR imaging (MR-DWI) for the detection of early liver fibrosis and to quantify the degree of fibrosis. Methods Rabbits developed of an experimental model of liver fibrosis by using CCl4 as well as rabbits for control were performed conventional MR scans and MR-DWI. According to histological fibrosis stage, the rabbits were divided into three groups: no fibrosis, mild/moderate fibrosis, and severe fibrosis/cirrhosis group. With different b values (300, 600, 1000 s/mm2), the results of DWI signal intensity (SI), apparent diffusion coefficient (ADC) and exponential apparent diffusion coefficient (EADC) among the three groups were compared. Results For no fibrosis, mild/moderate fibrosis and severe fibrosis/cirrhosis group, SI, EADC increased and ADC decreased gradually when using b values of 300 and 600 s/mm2 separately, the differences were statistically significant (P<0.01); while when b value was 1000 s/mm2, only SI increased as the severity of fibrosis progressed (P<0.01). When b value was 300 s/mm2, ADC and EADC were significantly different between all groups of fibrosis (P<0.01). Conclusion Our preliminary experimental results indicate that the diffusion in liver fibrosis reduced as the degree of fibrosis increased. MR-DWI may become an effective method for diagnosis and quantification of liver fibrosis when using an appropriate b value.
